0303692777 2015-06-10 Production Wells Violations Noted Follow-up, Complaint# 4986, Debbie Friedstrom. Myself and Tom Hill inspected the well to find there was fresh oil lying on the ground and floating on water lying at the north end of the tank battery area, also there is a new gas leak located at the north-east corner of the separator. The identification has not been posted at the tank battery area. Tom contacted Bob Barr Sr. and told him of the oil spill and the gas leak. A new compliance was issued for the oil spill and the gas leak.
0305567209 2015-08-20 Production Wells No Violations I was on location for a follow-up inspection in reference to compliance notice no. 2057909685. During my inspection, I observed a pool of oil at the north end of the tank battery next to the separator. There are used absorbent pad lying throughout the tank battery. The entire tank battery is stained with oil. I found five (5), oil stained, 5 gallon buckets with lids sitting next to the production tank. There is a 2 load line missing a plug. An identification sign is posted a production tank with no permit number. There is a Kubota excavator sitting on location next to the tank battery.
0385346818 2016-03-18 Production Wells No Violations I was accompanied by Jen Barret, Dan Martin, Mike Williams [ Attorney Generals Office ], Rick Anderson and Tom Hill [ODNR Division o Oil & Gas ]. Upon our arrival we found that the earthen dike had been breached on the north-east corner allowing the fluid inside the earthen dike walls to drain from the tank battery area into a crop field. I tested the remaining fluid inside the dike and in the crop field where the fluid had flowed with 1176 qauntab strips to find the chloride levels exceeded the limits of the chloride strips which was 6300 ppm chlorides. There was a new chart on the gas meter with the date of 3-10-2016 wrote on it. The chart indicated the well was in production at the time of the inspection. I took photos of the ditch in the earthen dike wall and inside the earthen dike walls.

0633005652 2018-02-27 Production Wells No Violations I, along with Supervisor Tom Hill, inspected this well location as part of on-going enforcement proceedings. As such, the following deficiencies had been noted previously: leakage of oil within the battery and lack of complete identifying signage. At the time of this inspection, we noted the following: a wellhead with plunger lift system and associated battery on a larger, rural parcel. The battery was comprised of two 100-barrel, steel production tanks, and horizontal separator. Fittings and connections at the wellhead were secure. There was oil and oil-stained ballast material throughout most of the tank containment area that remained from a prior release and had not been addressed. The chart box was not active and the well was not in production. The signage was faded and not legible. There had been no apparent effort to rectify the aforementioned deficiencies.

1017679487 2015-04-01 Production Wells No Violations Complaint# 4986 follow-up. I arrived at the gas leak location to find the gas is still leaking from the gas line. I contacted Don Goosman, he told me he had shut-in the line. He will investigate to determine how to shut the gas supply to the gas line off.

1254311537 2015-04-24 Production Wells No Violations Follow-up, Complaint# 4986, Debbie Friedstrom. The gas leak has not been repaired. I contacted Bob Barr Sr., he told me they attempted to repair the leak but the repairs did not hold. He then said he did not believe the gas line was his line. I told him I would make some calls to determine who owns the gas line. I traveled northeast of the leaking gas line off of State road and found a gas line owned by Dominion Gas Company and a gas line which was marked with a gas line marker showing C-Gas as the owner. There was two gate valves by the gas line marker owned by C-Gas. I contacted Dominion Gas, I spoke with Jeff Baker, he told me the line they owned off of State road was a 12 high pressure line and there were no gas wells in the area connected to the gas line. I then contacted Ken Kraeger , he was employed by C-Gas in the past and had knowledge of the gas line off of State road. He told me the gas line was sold to Big Sky with wells in the area. He said the gate valves I found was to a gas line that used to supply two factorys in Ashtabula City which have since gone out of business. Upon my request he voluntarily checked the gate valves to determine if they were closed. Don Kreager contacted me later in the day and told me the valves were open. He closed both valves which were four inch valves. I then contacted Don Goosman and told him of my findings of the gas line ownership and closing of the valves off of State road.

There was no sign of the line leaking gas. The identification has not been established and the well is not in production. I contacted Don Goosman, he told me they were able to repair the leaking gas line.

My inspection of the violations found there is still a active gas leak inside the earthen dike area on the east side of the separator. There is fresh oil floating on the water inside the earthen dike walls over 70 percent of the area inside the earthen dike walls. The identification for this well has not been posted at the tank battery area.
1446468389 2015-10-29 Production Wells No Violations My inspection found fluid has accumulated in the north end of the dike area, covering one third of the area inside the earthen dike walls and was about eight inches deep. The gravel inside the earthen dike walls was oil saturated about 80 percent of the area. There was gas still leaking from below the surface at the north-east corner of the dike area, two feet east of the separator.
1449846632 2015-12-02 Production Wells No Violations I conducted an inspection on this well and tank battery and found oily water accumulated within the north end of the dike area approximately six-to-eight inches deep. The gravel inside the dike was approximately 80 to 90 percent oil saturated. I observed five sealed 5 gallon buckets next to the production tank, within the dike present at the time of this inspection. Gas was still leaking from below the surface at the north-east corner of the dike. The wellhead valves were open. No issues at wellhead.

Oil staining was still present along the east side of the tank battery. Brine and iron staining was still present on the north end of the battery. All but the northern most end of the battery was dry.

The chart box was not active and the well was not in production. The signage was faded and not legible. There were no indications that any efforts to mitigate, clean-up, or fix the noted deficiencies had been undertaken.

Brine and free crude oil remain at the north end of the tank battery. Five gallon buckets of oil coated stone still remain in the battery. Oil stained and oil soaked soil and gravel remain in the center of the battery. No gas leaking next to the separator was noted at the time of this inspection. Chart in the chart box was illegible.

2057909685 2015-03-31 Production Wells Violations Noted Complaint# 4986, Debbie Friedstrom. I met with Chris Brockway at the tank battery location. We walked to the gas leak location, 500 south of tank battery area and 10 east of Freedom lane. There was a hole two feet in diameter which was full of fluid with gas venting through fluid at a constant rate. I contacted Don Goosman with Big Sky Energy. I told him of the gas leak, he told me he had no equipment to repair the leak right now but he would come out immediately to shut-in the gas line. This well is not in production at this time. ODNR Division of Oil & Gas records show the last reported production was in 2010, producing 1876 mcf of gas. Also the identification sign has no information posted on it. This well is in non-compliance. Remedial work required to get well into compliance: REPAIR GAS LEAK, POST CORRECT INFORMATION ON IDENTIFICATION SIGN, SUBMIT PRODUCTION REPORTS FROM 2010 TO PRESENT, PLUG WELL.
