There was no identification posted at the well or tank battery. The lease access was stable. Well has not been plugged.
0714371094 2017-04-25 Production Wells Violations Noted Complaint# 5593, David Chase. I inspected this well in response to a water well complaint. My inspection found the tank battery was equipped with one 210 barrel steel tank and one separator. There was no identification posted at the tank battery area for this well. The well had 4.5 casing at ground level with a swage to two inch with a valve which was closed. The surface casing was below the surface. The well was disconnected from the flow line. The division records show there has been zero production reported for this well since 2009. REQUIRED REMEDIAL WORK: Expose the surface casing head. establish identification at the tank battery area for this well. The well owner/operator shall submit a written plugging schedule to Jerry Kohl at Ohio Department of Natural Resources, Division of Oil & Gas Resources Management, 3575 Forest Lake Dr. Suite 150, Uniontown, Oh. 44685-8116 within 30 days, per OAC 1501:9-11-05 [ B ], and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

There was no identification posted at the well or tank battery. The lease access was stable. No plugging permit has been obtained by the company at the time of this inspection.
