0708181774 2019-04-10 Production Wells No Violations I conducted a compliance notice follow--inch casing swage with a two-inch valve. Valve was open. There was a one--inch casing swage with a regulator and a valve. valve was shut. The tank battery consisted of one 210- barrel production tank, and a mechanical separator. The wrong identification was posted at tank battery or at the well. The lease road was in stable condition. Compliance notices have not been resolved.

1543335986 2018-11-27 Production Wells Violations Noted I conducted an inspection on this well. The well was a 4.5 inch casing swaged into a 1 inch line with a regulator and ball valve. The valve on the casing was open but the 1 inch line valve was closed. There was a slight, audible gas leak at the swage and casing collar connection. No legible identification was located at the well. The production records for this well have been submitted as zero in 1988, and from 2011 to 2017. The tank battery location was comprised of one 210 BBL tank. There were no visible leaks observed at the tank battery. The dike around the tank battery was cut open on the south side, allowing standing surface water to flow into the tank battery and surround the 210 BBL tank. The identification posted at the tank battery did not contain the correct well information or current owner information. The lease access was stable. The well owner shall submit a written plugging schedule to Inspector Shannon Mullins at ODNR Division of Oil and Gas Resources Management, 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685 within 30 days. The well owner/operator will be notified if the plugging schedule is approved or denied. The Kenyon #1 well shall be plugged within 180 days. In the interim, the owner must correct the identification sign.
