1194986163 2018-10-09 Exempt Well Pre Transfer Violations Noted I conducted an exempt pre transfer inspection on this well. The well consisted of 8 5/8 inch casing and 4.5 inch casing that was swaged into a 2 inch line. The 2 inch line off of the 4.5 inch casing swage had a tee connection for 2 more 2 inch lines. One of the 2 inch lines was reduced to a 1 inch line with two regulators. That 1 inch line was connected to a drip bottle. The drip bottle had four, 1 inch poly lines conncected to it. Two of the four poly line valves off of the drip bottle, were open. The other 2 inch line from the well was plumbed into a 210 BBL steel tank, which was located approximately 10 feet from the well. There was a disconnected steel tank next to the 210 BBL tank at the well location. There was no other production equipment associated with this well. No signage was present at the well location. Lease access was stable. I contacted landowner Cynthia Pizzuto inquiring about how many dwellings were receiving domestic gas from this well. Ms. Pizzuto stated that two homes were receiving domestic gas from the well. I also discussed adding signage at the well location.

The well was approximately 830 feet from the nearest occupied dwelling and approximately 711 feet from the nearest unoccupied dwelling. The identification has been added at the well location. I contacted landowner Cynthia Pizzuto inquiring about how many dwellings were receiving domestic gas from this well. Ms. Pizzuto stated that two homes were receiving domestic gas from the well. The residence at 2262 Clay Street is receiving gas from the well for domestic use, and is located on the same parcel as the well. The other residence also receiving gas for domestic use from the well is not located on the same parcel as the well. Compliance notice closed for identification.
