0409732414 2015-10-05 Storage Well No Violations The operator has rigged up a vent stack on the annulus valve. Gas is venting from the stack. The operator stated they were reviewing logs to determine the corrective action for the well. The temperature log was inconclusive and did not show a leak in the production casing. An acoustic log showed noise events at 1900 feet and 800 feet per Dave Reynolds of Columbia Gas. The operator stated gas may be from another source other than this well.

1285964393 2015-10-15 Administrative Inspection No Violations Dave Reynolds of Columbia Gas contacted the inspector and stated that the corrosion log showed deterioration of the production casing in the well. The well was blown down and killed. A bridge plug was set in the well at 2755 feet. The well is presently out of service and the leakage has stopped. Dave believes the well is going to be plugged and abandoned.

The well bore was collapsing and had to be drilled out to the top of the 3.5 inch casing at 1904 feet. Plugs were set in 500 foot intervals from 1904 to surface. Only the surface plug circulated. No cement returned. The plug will be tagged tomorrow and finished.

A 2 iach bit was ran in the hole. The bit was plugged off running in the well and couldnt be cleared. The bit was pulled out. No cement was set today.

1513709742 2017-12-19 Plug / Plug Back No Violations The operator requested inspection to witness setting the surface plug in the well. The cement top was tagged at 120 feet. Gel and cottonseed hull LCM were pumped ahead of cement. Circulation was established. Cement returned to surface. Cement fell following pumping. A brush and stone bridge was placed in the well and the well was filled to surface.
1513795712 2017-12-20 Plug / Plug Back No Violations The top of the plug is approximately 1 foot below the casing top. Mac Oil is on site grading location. Water has to be hauled from the pit and a water tank. Mac is going to cut the well off and install the plate on the casing.

No fluid could be heard and the well was observed for 10 minutes and no fluid emerged. Gas is surging up and down in volume. Well tender Casey Cole was contacted and advised to check the well. The tender for this area is on vacation. Casey is going to pressure gauge the annulus and see if it builds storage pressure.

On attempt to sting in the retainer, the well was still circulating. On trip out, the stinger was found to be damaged. A 40 sack cement plug was pumped inside the 3.5 inch on top of the cement retainer. Baker Hughes ran free point tools in the well. An attempt was made to tag the cement top but the cement was not set enough to tag. The 3.5 inch was freepointed at cut at 1904 feet.
1955759617 2016-09-29 Storage Well No Violations ID is posted and the location is maintained. The well is disconnected from the separator. The wellhead has been removed. There is a swage on the production casing. There is a valve on the swage with a bullplug in it. The well remains killed with a bridge plug above the Clinton Sand. There is no leakage from the well. The operator is planning to plug and abandon the well.
