0232806907 2018-03-15 Production Wells No Violations The well is equipped with open 4.5 inch casing at ground level. An odor of gas was observed above the well. There is flowline, an electric pole and an old pit at the location. There are 2-100 barrel tanks at the road entry and a 50 barrel tank near permit 2-1983. The inspections entered in 2005 and 2015 for this well correspond to permit 2-1993. This well had not been located at that time. The well locations need to be surveyed to confirm permit numbers. Ben Harpster of the Orphan Program requested inspection of the well following inquiry by the landowner, Dan Stapp. Following Chiefs Order 88-01 and enforcement case 89-A16, the well was referred to the orphan program for plugging. Harold Moravy of the Division referenced an orphan file in 2005. Ben stated he would check the orphan records for the file.
0924529839 2015-04-08 Production Wells No Violations The well has a well head on it that is open. The well has been stripped of rods and tubing. Upon approach to the well odors of gas were noticed. A concrete block was sitting on the casing. Upon lifting the block a very strong odor of oil and gas was emitted from the well. There is steel flow line present and an electric pole with a disconnect and timer on it near the well. Wires from the meter and main disconnect are down and tangled in trees. There is no power from the main disconnect. The meter has been removed. Bond was forefeited on this well and it was referred to the orphan program. There are 2-100 Bbl tanks at the lane entry and a 50 Bbl tank near permit 2-1983. An old pit is near the well.
1111594368 2005-03-21 Orphan No Violations Sherry Young request. Well refered to I&o program by Attorney Generals office. Old case # 89-A-16. Well is equipped with 4.5 well head on 5.5 casing. Well is leaking gas at 10% lel. Old production lines above ground needs removed during plugging. Old electric lines on site needs removed during restoration of site. Berea well 700 in depth.Drilling pit needs closed. Well has not produced since 1960s. Wet area and culverts will have to be installed to cross stream. Heavily wooded area.
