0518756095 2014-11-05 Production Wells Violations Noted The operator contacted the Division and advised that they had a buried tank over flow after a pump in the tank failed. They also contacted Ohio EPA and obtained incident number 1411-03-2291. Fluid escaped from the tank and into an earthen containment area. Fluid then leached through the soil and into an adjacent stream which is a tributary to Redhaw Creek. The operator immediately began containment and cleanup of the spill. A siphon dam was constructed at the upstream end of the culvert under County Road 175. An absorbent boom was placed behind the siphon dam and absorbent pads were placed behind the boom. Personnel were using additional pads to clean up oil from the water surface and creek bank. Oil was contained behind the dam. An additional boom was placed to contain oil at the creek bank where it was seeping out. In the morning the operator is bringing excavating equipment to remove contaminated soil. In addition, the vault is going to be replaced by a brine tank inside concrete containment above ground. The operator estimates 3 gallons of oil in the creek. Complete clean up of spilled oil and contaminated soil immediately. Cease use of and remove the vault and place all tanks inside concrete containment to prevent any future spills from reaching the waterway. Complete tank battery renovations by 11/26/14.
0867393106 2014-04-02 Production Wells No Violations Brine is leaking from a fitting on the flow line. The operator was advised to make repairs. ID needs to be posted. The well is in production, equipped with a pump jack, rods and tubing.
0885704459 2015-02-03 Production Wells No Violations The operator has continued to maintain the brine vault while waiting for delivery of a new fiberglass tank for brine storage. Fluid is being pumped out of the vault to prevent buildup. The tank was originally to be on site in January but has been delayed at the manufacturer. The operator contacted the tank company today and was told the tank is near completion. Once the tank is complete, delivery will be scheduled. The operator would like to install the tank when air temperatures are above 40 degrees. The compliance notice is extended through the end of February.
1415384453 2014-11-06 Production Wells No Violations The operator excavated out contaminated soil and took it to their yard for on site remediation. The earth containment was constructed with a clay liner which was damaged when a new tank battery with concrete containment was installed. Oil escaped the earth containment where it met the edge of the concrete containment, flowed under the concrete and entered the creek from a fissure in the bank. The operator will have a new tank to replace the vault in 2 weeks. The new tank will be placed inside the concrete containment and the transfer pump that failed causing the vault to overfill is also going to be placed inside the concrete containment structure. They continue to clean up oil from the stream behind the tanks down to the culvert at County Road 175. The oil is contained upstream of the road culvert.
1417789525 2014-12-04 Administrative Inspection No Violations Randy Matheny of Deep Resources stated that the tank is ordered and should arrive around the third week of December 2014. All clean up has been completed at the tank battery. The operator believes that once the tank arrives they should have it plumbed in shortly thereafter. The compliance notice is extended until Jan. 31, 2015.
1808632181 2015-02-26 Production Wells No Violations The well is equipped with a pump jack and is in production. ID posted. There are 2-200 Bbl tanks and a vault in the tank battery. The vault is due to be replaced with an above ground tank inside the concrete containment structure.
1832418521 2018-01-03 Production Wells No Violations Met with the operator at the company shop. The operator previously planned to remove the brine vault from the tank battery as a result of an overflow and spill in 2014. Ownership of the company changed. The previous owners planned to install a fiberglass tank and remove the vault. The new owners dont want to use a fiberglass tank and plan to install a steel tank in the tank battery. There is currently no timetable to remove the vault and replace it with an above ground tank.
