1322541924 2018-03-16 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain one properly labeled wellhead. Well is equipped with a plunger lift system. Wellhead pressures read as follows: Surface 200 psi, intermediate 220 psi, tubing 725 psi, production 825 psi, and flowline 660 psi. Production equipment includes one GPU, one sand seperator, and two 400 barrel steel produced water tanks. Tanks are in steel containment. Eight frac tanks are also onsite. Site meets requirements of ORC 1509.072A for preliminary restoration.
1835679101 2016-02-17 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

2 400 bbl. Production tanks 1 Producing well 1 Sand separator

1 Horizontal heater-treater. There was approximately 4 to 5 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.
