1496274604 2013-10-17 Storage Well No Violations On the day of my inspection I witnessed that the valves were open and the well identification was faded. I made a call to Rick Warne of Columbia Gas and he stated that he would fix the identification and meet with me on Monday October 21, 2013.

2032699902 2018-10-25 Storage Well No Violations On the day of my inspection I found that the well was equipped for gas storage. The lines were connected and the valves were open. The well was not equipped with a gauge to show pressure. There were no leaks around the wellhead or lines. The identification was incomplete. The API permit number was not listed and the emergency contact phone number had been painted over. I was able to barely make the number out so I called and found that it was the emergency phone number. I wrote the correct permit number and phone number on the ID sign.

The well was not equipped with a gauge to show pressure. Correct, legible identification was posted. There were no leaks around the wellhead.
