0885813692 2013-09-04 Production Wells Violations Noted This well consists of a well head and 2 7/8 tubing with a valve on top. Unable to locate any tank battery. Well showed state minimum production in 2010, and since 2004 no other production at all. This well is to be plugged in accordance with state regulations. Failure to comply may result in a Chiefs Order to plug and abandon. Additionally, wellhead is to be identified in accordance with state law.

0962377072 2017-10-27 Production Wells No Violations Inspected production well on 10/27/2017 to follow-up on a previously-issued compliance notice. The well is equipped with a threaded wellhead with a swedge to ~2 3/8 tubing, and 1-master valve. No tanks were noted nearby, nor was any well identification posted. The landowners daughter, Deb Marthey, indicated that the well had not produced in many years (which was confirmed with DOGRM production records). These issues were discussed with Jeff Harmon (MFC Drilling), who indicated that they had planned to plug the well several years ago, however due to a dispute with the landowner regarding well access routes, they did not do so. He indicated that they would promptly apply for a plugging permit, and requested DOGRM assistance in coordinating acceptable wellhead access with the landowners. The situation will continue to be monitored any assistance provided to ensure the well is promptly plugged, and the compliance issues corrected.
