| On my inspection today I found that the compliance notice has still not been met. The pump still has no motor on the pump jack and the well is still not producing. I contacted Jim and let him know if this were to continue there would be a chiefs order to fix the well. He informed me he would get a motor on the pump in the next week or so.

| The inspection this day revealed that this well is equipped with a pumping unit, a 4 1/2 inch diameter wellhead, 1 1/2 inch diameter production tubing, and 2 inch diameter steel production lines. The valves on the wellhead are closed at the time of inspection. There is a single 100 barrel steel tank and a 50 barrel steel tank at the entrance to this well. After reading previous reports Jim Heughey of Killbuck claims it is a tank from a Chipco well that was plugged. There is no identification on the tank but, there is a meter box with a chart in it for one of Killbucks other wells not this one specifically. The chart in the box is from 10/15/16. There has been no resolution to the idle well issue on this well and this well is in nonproduction at the time of inspection.

| Well is currently shut in and incapable of production. There is one pump jack with completely rusted rod. Wellhead is equipped with 4.5 and 8 casing and 2 tubing. Pumpjack has no motor. There was a slight odor of gas at wellhead. There is no ID at well or at tank battery. I called number listed for Killbuck Creek Oil Co and left a message explaining that the well needed to be produced or plugged and an ID posted. 03/01/2013: Talked with operator and he said they had 200 psi on the well and were able to get small amount of gas. They are planning on bringing a pumping unit out sometime in April when the weather is better and try to get well back into production.

| I was at this well to do a follow-up inspection on it. The pumping unit has been leveled out and has new boards under it. The well is still not capable of production at this time though because there is no motor on the well. I contacted Jim Huey to see when he plans on getting the motor on it. He said he would have it on in the next 2 weeks. He said the motor he had on it seized up so he has to do some work to it. I told him to contact me when he puts the motor on so I can come back to the well.

Jim Huey contacted me to let me know he put a new motor on the pumping unit and the well is back in production. During my inspection I found the well is capable of production with a new motor on the pumping unit and the production valves are open. The well is now in compliance.
