| At the time of inspection the drill rig was on site and placed on containment.  The erosion and sedimentation plan was on site.  The various pit located onsite appear to be in sound condition and had the required 2' of freeboard.  The Department suggest the Operator continue to inspect the site weekly and after significant precipitation events to ensure the stability of the site is maintained.

| Inspection the result of a complaint received by the Department on 7/10/2013 at approximately 1:00pm. The adjacent landowners issue stems from excessive runoff water on the property after a significant precipitation event.  At the time of inspection the Department observed saturated ground behind the complainant home and garage, also observed were traces of sediment deposits.  Upon further inspection of the well site the Department observed a compromised silt sox BMP directly  behind the complainants home and garage.  The Department notes that this was most likely the reason and source of the excess water.  The Department suggests the Operator submit metrological/rain fall data for July 10 2013.  The Department suggest that the Operator also submit weekly site inspections reports from 6/10/2013 to the date of this incident for the Departments review.

| Follow-up inspection from a complaint filed with the Department on 7/10/2013.  During the inspection the Department observed several areas that the Operator fixed following the precipitation events in July.  The Department notes that the Operator has filed meteorological data supporting the extreme rainfall event on July 10 2013, which resulted in the BMP failure noted in the July 10 2013 inspection.  At the time of inspection the well appeared to be drilled, however the wells donï¾’t appear to be in production.

| At the time of inspection the well was in the process of bring stimulated.  The E&S plan was onsite and appeared to be installed as  outlined in the plan.  No sign of accelerated erosion and sedimentation in relation to the well pad or access road at the time of inspection. The Department notes that the drill pit located onsite has been back filled and the stock pile from which the material came to backfill the pit has been hydro seeded and appears to be stable.  The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.

2183275 Primary Facility 07/10/2013 Complaint Inspection No Violations Noted Inspection the result of a complaint received by the Department on 7/10/2013 at approximately 1:00pm.

2197076 Primary Facility 08/19/2013 Routine/Complete Inspection No Violations Noted Follow-up inspection from a complaint filed with the Department on 7/10/2013. During the inspection the Department observed several areas that the Operator fixed following the precipitation events in July. The Department notes that the Operator has filed meteorological data supporting the extreme rainfall event on July 10 2013, which resulted in the BMP failure noted in the July 10 2013 inspection. At the time of inspection the well appeared to be drilled, however the wells don’t appear to be in production.
2216370 Primary Facility 10/24/2013 Routine/Complete Inspection No Violations Noted At the time of inspection the well was in the process of bring stimulated.
2228551 Primary Facility 11/11/2013 Routine/Complete Inspection No Violations Noted At the time of inspection the well was stimulated. The E&S plan was onsite and appeared to be installed as outlined in the plan. No sign of accelerated erosion and sedimentation in relation to the well pad or access road at the time of inspection. The Department also suggests the Operator add the approved OG071 to the mail box on site. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
2272552 Primary Facility 05/21/2014 Routine/Complete Inspection No Violations Noted At the time of inspection well appeared to be drilled, stimulated and equipped for production. The Erosion and Sedimentation Plan was on site. No accelerated erosion or sedimentation observed in relation to the access road or well pad area. The perimeter E&S BMPs appear to properly installed and functioning properly. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site in maintained.
2326220 Primary Facility 11/25/2014 Routine/Complete Inspection No Violations Noted At the time of inspection well appeared to be drilled, stimulated and equipped for production. The Erosion and Sedimentation Plan was on site. No accelerated erosion or sedimentation observed in relation to the access road or well pad area. The Operator appeared to be in the process of backfilling the flow back pit located onsite. The perimeter E&S BMPs appear to properly installed and functioning properly. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site in maintained.
