2040647 Primary Facility 2012-02-06 Routine/Complete Inspection No Violations Noted Site inspection was performed as a result of a request received from the operator for an extension to the 9-month restoration timeframe due to unusually high precipitation totals which have resulted in conditions that are not conducive to effectively restoring the well-site. Drilling was completed on 06/20/11. 9-month restoration window expires on 03/20/11 Well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. Pit located onsite. Pit contained fluids which were being removed via tanker truck. Pit had more than the required two feet of lined freeboard. All site and access road erosion controls were found to be functioning properly. It was noted that a rock lined drainage ditch conveying runoff from between the well pad and pit located onsite required routine maintenance below the well pad.
2068013 Primary Facility 2012-05-15 Routine/Complete Inspection No Violations Noted At the time of inspection the site appear to be in the beginning phases of restoration. The E&S plan was onsite and the BMP's were installed and properly maintained. The flow back pit adjacent to the well pad appears to be regarded and backfilled.

The well apparatus and brine tanks located onsite appear to be in sound condition. At the time of inspection the site had been regraded, seeded, and mulched. The vegetation is growing nicely at this time but does not meet the 70% uniform vegetative criteria needed to complete restoration. During the inspection the department notes that the erosion and sedimentation plan was onsite and all BMPs were still installed as outline in the E&S plan. All BMPs appear to be in sound condition and functioning properly. No accelerated erosion or sedimentation observed in relation to the well pad or access road at the time of inspection. The department suggest the operator continue to monitor the site after significant precipitation events and weekly to ensure the integrity of the site is maintained.

2349176 Primary Facility 2015-03-02 Incident- Response to Accident or Event In Compliance with Policy Inspection in response a 5 gallon brine spill reported by the Operator on 2/28/2015 @ 13:47. At the time of inspection the well site was covered in a thick layer of ice. The Operator indicated that spilled brine was immediately scrapped from the ice. The Department requests the disposal manifests associated with disposal.
