1962072 2011-02-24 No Violations Noted Site preparation is underway. Operator is in the process of adding rock to the access road and well pad. Site ID plan and E&S plan located. There are no signs of erosion or sedimentation on the well site or access road. Drilling will begin next week.
1963795 2011-04-01 No Violations Noted Drilling 26" hole at 257' of proposed 280' at time of inspection. Pits lined and freeboard maintained. E/S controls in place and functioning as per available plan.
2076893 2012-06-15 No Violations Noted At the time of inspection the well was drilled and stimulated. The Operator indicated that the well was being equipped for production. The associated brine tank and secondary containment appeared to be in sound condition. No accelerated erosion and sedimentation observed in relation to the well pad area or access road. The E&S plan was onsite, the BMPs were properly implemented and maintained. The Department suggest the Operator continue to monitor the site on a weekly basis and after significant precipitation events to ensure E&S remains at a minimum.
2087335 2012-07-30 No Violations Noted Inspection of pit to evaluate pit condition prior to restoration extension. At the time of inspection the Department notes that the pit was lined and appears to be in sound condition. The pit had the required 2 feet of freeboard. The Department did not observe accelerated erosion or sedimentation in relation to the well pad or access road associated with the pit. The Department note the site has been temporary stabilized and all E&S controls appear to be in sound condition and properly installed and maintained. The Department suggests the Operator continue to monitor the pit and surrounding area weekly and after significant precipitation events to ensure erosion and sedimentation is kept at minimum. At this time the Department extends the restoration requirements of the Aikens Pit ESX 10-129-009 until the June 30 2013
2101711 2012-09-20 No Violations Noted At the time of inspection the well appeared to be drilled and equipped for production. The well apparatus, brine tanks and heating units appear to be in sounds working condition. The Departments notes the Operator was actively working toward restoring the well pad area as outline in the restoration plan. At the time of inspection the Erosion and Sedimentation plan was onsite and appeared to be implemented. All BMPs were installed and functionally properly. The Department suggest the Operator continue to monitor the site weekly and after significant precipitation event to ensure the stability of the site in maintained.

Mechanical integrity of operating wells). 9 5/8” x 5 1/2” annulus closed and monitored (12-04-12 = 120 psig). Requested from Operator rep S. Walker 9 5/8” casing shoe test. 20” x 13 3/8” and 13 3/8” x 9 5/8” annuli cemented to surface and open to atmosphere (covered w/pea gravel). Well located on well pad containing (8) eight wells, (8) eight production units, and (8) eight produced fluids tanks. Well pad site reclaimed.
