1960778 2011-03-22 No Violations Noted Water truck filling frack pit at time of inspection. Frack pit lined and freeboard maintained.

Well shut in on 4 1/2" x 2" swage. 7" x 4 1/2" annulus closed. E/S controls in place and functioning as per available plan.

1969376 2011-04-08 No Violations Noted Drilled, not equipped for production. Site ID sign and E&S plan located. Pit is on-site / in sound condition. An E&S concern was noted while on-site-

The culvert closest to the well site is draining directly into the silt fence on the lower side of the site. There is no where for the water to go. The operator was contacted and it was decided that a section of the silt fence will be replaced with a rock filter and additional rock protection will be added around the culvert.

1969384 2011-04-22 No Violations Noted Met with operator representative Harry Mills to check out improvements made to the E&S controls.

The operator has added additional rock protection to the culverts. IN addition, a section of the silt fence has been removed and replaced with a rock filter. The improvements look stable and should operate effectively.

Harry Mills also informed me that next week (weather permitting) they will add an additional load of clean, durable rock to further stabilize the area.

At this time, the site looks good. The operator has addressed all previously mentioned concerns.

Well equipped with separator, drip, meter, and produced fluids tank. Inspection conducted w/OGI D. Catalano. 7" x 4 1/2" annulus open and piped for production through regulator into separator. Operator did not install a working pressure gauge that can be inspected by the Department when utilizing surface casing, or any casing functioning as a water protection casing, as production casing. Well not in compliance with Chapter 78. Sections 78.83.(a)(2), 78.83.(j), 78.83b.(a)(3), and 78.86.

2047162 2012-03-02 No Violations Noted Drilling had been completed on 03/10/11. Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. Site had been restored and the vegetative cover met the 70% uniform coverage criteria. It was noted that the well was located in a small pasture field.
2053761 2012-03-26 No Violations Noted Well appears to drilled and equipped for production. Well apparatus and brine tank located on site appear to be in sound condition. No observation of accelerated erosion on the well pad or access road noted. The site has been restored.
