2574900 Primary Facility 2017-03-23 Routine/Complete Inspection No Violations Noted On 3/23/17 at 1155 hrs I conducted a routine inspection of the Signor 578 well site. It was sunny and 29 degrees F. No one was present during the inspection.

The site, which consists of the access road and a single-well pad, was idle. The 1V well has been drilled and was placed on inactive status on 5/7/14. The site was snow covered, with the exception of the topsoil stockpile and the cut and fill slopes surrounding the pad. The topsoil stockpile is permanently stabilized. Vegetative cover on the pad slopes was somewhat sparse and areas not vegetated were rocky.

An ESCGP-2 was issued on 10/1/15; this replaces the original ESCGP-1, issued on 8/27/10, which authorized construction of the existing well pad. SWEPI requested a two-year extension to the restoration requirements on 6/30/15; the extension was granted upon issuance of the ESCGP-2.

No violations were cited during this inspection. I concluded the inspection at 1250 hrs. TLT
