2643353 Primary Facility 2017-10-02 Routine/Complete Inspection No Violations Noted On October 2, 2017 I conducted an inspection of the MOR 30 well site located in East Finley Township, Washington County. Mike Farrah and John Hardcastle were present from CNX Gas Company, LLC. Conductors were being drilled for the L(125-28276), M (125-28278), and N (125-28277) wells.

The conductor cuttings had a dry, brown appearance. There was no odor of brine or petroleum. The only rig on the well site was the augor for drilling the conductors. The area where the cuttings are being disposed may need to be hydroseeded to establish vegetation. The disposal of uncontaminated rock and soil from drilling the conductors at this site appears to be in accordance with 78a.61(b). CNX Gas is recommended to ensure Consol owns the property. Otherwise, the requirement to notify the landowner within 10 days of the disposal under 78a.61(e) would apply. No violations noted.

2803743 Primary Facility 2018-11-14 Routine/Complete Inspection No Violations Noted On November 8, 2018, the Department performed an inspection of the CNX MOR40 well pad site. Present at time of inspection was William Roupe and Spencer Johnson (CNX). The site is located in East Finley Twp., Washington Co. The inspection was a compliance evaluation related to reported slide, which has left the LOD, by the operator, and violations on 9/6/18.

The operator has restored the site to the interim restoration plan and has constructed the PCSM BMPs. A slope failure down-gradient the well pad PCSM (Infiltration Berm) has caused the PCSM to fail and has left the approved LOD. The saturated soils at the berm have failed causing a slide approximately 220'x50' and has left the LOD. The trees in the area are also failing and toppling over. The operator was given guidance to submit a modification to the department for expansion of the LOD to encompass the slide repair area. The operator will also need to complete a geotechnical investigation and report for repair of the slide and stability of the site. Th
