2633262 Primary Facility 2017-08-22 Routine/Complete Inspection No Violations Noted On August 22, 2017, the Department performed an inspection of the CNX MOR 31 well pad site. Present at time of inspection was Dave Fredericks (CNX). The site is located in East Finley Twp., Washington Co. The inspection was a compliance evaluation inspection related to violations noted on 7/26/17.

The operator has repaired the failed E&E BMPs and has remediated and recovered soils which had left the LOD. The operator is finishing construction of the well pad and access road. All the perimeter E&S BMPs are in place and are functional. The operator has asked to make a minor modification to the ESCGP2 permit to expand the excess soil stockpile. The minor modification was reviewed and approved by the Department in the field.

6 wells have been permitted for the site. Site is still under construction so none of the wells have been drilled.

2640542 Primary Facility 2017-09-18 Routine/Complete Inspection No Violations Noted On September 18, 2017, the Department performed an inspection of the CNX MOR31 well pad site. Present at time of inspection was Dave Frederick and Alex Molnar (CNX). The inspection was a routine inspection during construction of the site related to a request by the operator for a pre-operational inspection of the site prior to the start of drilling activity.

The operator has installed all the perimeter E&S BMPs which are in good condition. The operator has completed construction of the access road and well pad. The operator has stabilized the well pad fill slopes and site with hydro seeding. The operator was stoning the well pad and installing cellar tech well cellars at time of inspection.


6 wells have been permitted for the site and the operator plans to set conductor on these wells in the near future.

2643179 Primary Facility 2017-10-02 Routine/Complete Inspection No Violations Noted On October 2, 2017 I conducted an inspection of the MOR 31 well site located in East Finley Township, Washington County. Mike Farrah and John Hardcastle were present from CNX Gas Company, LLC. Mr. Hardcastle said they had received 4 out of 7 well permits for the site so far: F (125-28269), G (125-27633), N (125-28326), and P (125-28327).

The conductors have been drilled to 100' and 26" casing set. They were grouting to surface on one of the wells at the time of the inspection. The conductor cuttings had a dry, gray, powdery appearance. There was no odor of brine or petroleum. The only rig on the well site was the augor for drilling the conductors. The conductor cuttings were being disked into the top of the soil stockpile with the blade of a bulldozer. The soil stockpile had been hydroseeded but no vegetation was growing. The area where the cuttings are being disposed may need to be hydroseeded again to establish vegetation. The disposal of uncontaminated rock and soil from drilling the conductor

2747447 Primary Facility 2018-06-28 Incident- Response to Accident or Event Violation(s) Noted CNX reported a location fire incident with the first report of fire at ~ 9:30 PM Wednesday, 27 June. Fire was reported to West Finley Fire Department, and was extinguished by ~ 12:30 AM Thursday 28 June.

Initial reports indicate 7 pump trucks, a man lift, a 4000 Gallon diesel fuel truck and two acid tractor trailers (believed to be 5000 gal tanks carrying approx. 4000 gal of 28% HCl acid each) and a safety shower trailer were involved and seriously damaged by fire.

Frac crew was pumping stage 16 of 48 on the "F" well when the fire broke out.

Operator is currently performing root cause analysis of the fire and will get back to the department with results as soon as they are available.

DEP WQS have also inspected location and are following up on contamination issues due to containment failures. On site investigation confirms contamination, with operator calculating amounts unaccounted for and reporting to department.

2750295 Primary Facility 2018-06-28 Incident- Response to Accident or Event Violation(s) Noted On June 28, 2018, the Department performed an inspection of the CNX, MOR31 well pad site. Present at time of inspection was Adam Oftsun, Joel Keller, Eric Peterson (DEP), Spencer Johnson, Jason Harding, Pat Carfagna, and Andrew Perine (CNX). The site is located in East Finley Twp., Washington Co. The inspection was an incident inspection related to a reported CNX fire during hydraulic fracturing operations.

The fire was first reported at approximately 9:30 PM Wednesday, 27 June, by the operator. Fire was reported to West Finley Fire Department, and was extinguished by approximately 12:30 AM Thursday 28 June.

Initial reports indicate 7 pump trucks, a man lift, a 4000 Gallon diesel fuel truck and two acid tractor trailers (believed to be 5000 gal tanks carrying approx. 4000 gal of 28% HCl acid each) and a safety shower trailer were involved and damaged by fire.

The site is located in East Finley Twp., Washington Co. The inspection was an compliance evaluation inspection related to a reported CNX fire and fluid release during hydraulic fracturing operations on June 27, 2018

2780114 Primary Facility 2018-09-24 Incident- Response to Accident or Event No Violations Noted On September 24, 2018, the Department performed an inspection of the CNX, MOR31 well pad site. The site is located in East Finley Twp., Washington Co. The inspection was an incident inspection related to a reported brine release by the operator.

The operator reported a release of approximately 15 gallons of brine to the well pad. The operator noticed stained stone and soil near the production area of the well pad. A pump was being used to transfer production fluid on the well pad, during flowback of the wells recently completed, and a valve on the pump was observed leaking. The leaking production fluid mixed with rain water in the secondary containment for the pump and overflowed onto the production pad. The operator has shut down the pump removed the brine/precipitation from the secondary containment using a vac truck. The visually impacted stone and soil on the well pad was removed by the operator and will be disposed of at an appropriate facility.
