The site is located in East Finley Twp., Washington Co. The inspection was a routine inspection during construction of the site.

The operator has installed the perimeter E&S BMPs. Two sections of stacked silt sock near the entrance to the site have failed and have become damaged. The operator indicated that Filtrex was coming to the site to repair and improve the failing silt sock. No sediment appeared to have left the site during the damage to the silt sock. The completed slopes of the access road where being stabilized with seed and mulch at time of inspection. Construction of the access road is close to being completed and the operator has started construction of the well pad.

12 wells have been permitted for the site, but have not been drilled yet.

2616015 Primary Facility 2017-06-15 Routine/Complete Inspection No Violations Noted On June 15, 2017, the Department performed an inspection of the EQT Ealy well pad site. Present at time of inspection was Jeff Osburn (EQT). The site is located in East Finley Twp., Washington Co. The inspection was related to a request by the operator for a pre-operational inspection prior to the commencement of drilling on the site.

The Act 9 site ID sign was in place and legible.

The access road to the well pad has been constructed. The operator has completed construction of the well pad and fill slope. During the inspection the operator was in process of stoning the pad in preparation of drilling on site. The operator has started construction of the freshwater impoundment for the site.

All perimeter E&S BMPs have been installed for the site and are functional.

12 wells have been permitted for the site, but have not been drilled yet.

Present at time of inspection was Walter Nelson and Bill Balog (EQT). The inspection was related to a request by the operator to meet on site to discuss site restoration.

The operator plans to remove the E&S BMPs (silt sock) along the well site access road to allow for the installation of a fence. The landowner has requested that a fence be built along the access road to accommodate grazing for his cattle. The landowner has also requested a temporary tractor access road near the freshwater impoundment to grant access to a west pasture field near the impoundment. The site is table with greater than 70% vegetative cover so removal of the silt sock would be permitted. The Department also reviewed the location of the temporary access road and approved the minor modification to ESCGP2 permit with a redline change on the E&S plan.

2814422 Primary Facility 2018-12-07 Routine/Complete Inspection No Violations Noted On December 7, 2018, the Department performed an inspection of the EQT Ealy well pad site. Present at time of inspection was Matt Parker (EQT consultant). The site is located in East Finley Twp., Washington Co. The inspection was related to a reported release Diesel based drill mud to the well pad by the operator.

On December 4, 2018 the operator reported a 20 gallon release of diesel based drill mud to the well pad. The operator reported that during loading the hole with drill mud to commence drilling operations on the 593332 well (125-28048), the cuttings processing area was overloaded with drill mud and the drill mud released from the flare stack to the well pad. The operator reported that the visually impacted soils were removed and disposed of in an empty cuttings half round to be disposed of during drill cutting disposal.

The site is located in East Finley Twp., Washington Co. The inspection was related to a request by the operator for a minor modification to the ESCGP2 permit.

The operator has requested a minor modification to the ESCGP2 permit to construct a fresh water pipeline, and gathering pipeline within the well pad LOD to connect to the Equitrans NIPI-S003 pipeline site. The minor modification was reviewed and approved by the Department in the field. The modified ESCGP2, E&S plans were redlined by the Department and left in the mailbox on site as the official plans for the site.
