The 13 3/8” casing was set at 935 ft. on 5/20/16 and cemented to surface with 30 bbls returned.  Coal was found from 885 ft. to 891 ft.  Two (2) baskets were set 765 ft. and two (2) baskets were set at 800 ft.  The 9 5/8” casing was set at 2573 ft. on 5/22/16 and cemented to surface with 40 bbls returned.  A Casing Annulus Packer was set in the 9 5/8” casing string at 1788 ft.  KOP was at 2720 ft.  TD was at 5910 ft. on 5/26/16.
 
 
 
Note:  The Department informed the operator that setting baskets 120 ft. above the coal was unacceptable as well as the use of four (4) baskets on one coal-protective casing string.

TD at 20,522 ft. on 1/25/17.  Set 5 ½” casing at 20,512 ft.on 1/27/17.  Cemented casing on 1/28/17.  There were no issues when starting cement job.  Pressure increased and operator did not get returns.  Wireline showed TOC at approximately 13,200 ft.  Casing was cut at 12,627 and pulled out of wellbore.  On 2/5/17 operator picked up sidetrack assembly and started sidetrack at 11,931 ft.  Currently at 14,615 ft. and drilling ahead.

| Today I conducted a routine inspection at the Cogar well pad and the following was observed.
 
 
 
Proper signs were displayed identifying the site. EQT currently has a rig on site and is drilling 1 of 6. The well heads onsite are contained inside subsurface vaults and covered with equipment for the drilling process and could not be visually inspected. The production tanks were in good condition no leaks , rust or defects observed. Secondary containment for the site was intact and maintained.
 
 
 
The site had some stabilization issues on the SW side of the access road.
 
I there was some slides forming inside and outside the LOD.  I spoke with Brian Morris from EQT about the issue and we arranged a site inspection on 3/13/18 at 1:30pm to discuss the plan for stabilizing that area.

The well heads onsite are contained inside subsurface vaults and covered with equipment for the drilling process and could not be visually inspected. The production tanks were in good condition no leaks , rust or defects observed. Secondary containment for the site was intact and maintained.

The 13 3/8” casing was set at 935 ft. on 5/20/16 and cemented to surface with 30 bbls returned. Coal was found from 885 ft. to 891 ft. Two (2) baskets were set 765 ft. and two (2) baskets were set at 800 ft. The 9 5/8” casing was set at 2573 ft. on 5/22/16 and cemented to surface with 40 bbls returned. A Casing Annulus Packer was set in the 9 5/8” casing string at 1788 ft. KOP was at 2720 ft. TD was at 5910 ft. on 5/26/16.

Note: The Department informed the operator that setting baskets 120 ft. above the coal was unacceptable as well as the use of four (4) baskets on one coal-protective casing string.

Moved onto this well on 1/13/17 and resumed drilling at 5925 ft. MD. EOB was at 8774 ft. MD (7905 ft. TVD). Currently at 18,451 ft. MD and drilling ahead.

TD at 20,522 ft. on 1/25/17. Set 5 ½” casing at 20,512 ft.on 1/27/17. Cemented casing on 1/28/17. There were no issues when starting cement job. Pressure increased and operator did not get returns. Wireline showed TOC at approximately 13,200 ft. Casing was cut at 12,627 and pulled out of wellbore. On 2/5/17 operator picked up sidetrack assembly and started sidetrack at 11,931 ft. Currently at 14,615 ft. and drilling ahead.

Note: Operator failed to notify Department of defective, insufficient or improperly cemented casing.

TD sidetrack at 20,577 MD (7690 ft. TVD) on 2/12/17. The 5 ½” casing was set at 20,567 ft. on 2/14/17. Attempted to cement casing on 2/15/17, failed to get returns. Ran wireline tractor which failed. Brought 2nd wireline tractor to location which froze. Currently TIH with wireline to run cement bond log and will try to determine TOC.

Attempt to cement 2nd 5 ½” casing (1st sidetrack) on this well failed. Operator cut drill pipe on 1st sidetrack at 10,522 ft. MD on 2/20/17. Operator started 2nd sidetrack at 9870 ft. MD on 2/22/17. EOB is at 8974 ft. MD. Currently at 10,932 ft. and drilling ahead.

Note: Operator failed to notify Department of defective, insufficient or improperly cemented casing on 2nd production casing, after 1st sidetrack, after being made aware to notify on failed cement job on first production casing string on this well.
