2755437 Primary Facility 2018-07-11 Routine/Complete Inspection No Violations Noted On July 11, 2018, the Department performed an inspection of the CNX, MOR31 well pad site. The site is located in East Finley Twp., Washington Co. The inspection was an compliance evaluation inspection related to a reported CNX fire and fluid release during hydraulic fracturing operations on June 27, 2018

The fire was first reported at approximately 9:30 PM Wednesday, 27 June, by the operator. Fire was reported to West Finley Fire Department, and was extinguished by approximately 12:30 AM Thursday 28 June.

Initial reports indicated 7 pump trucks, a man lift, a 4000 Gallon diesel fuel truck and two acid tractor trailers (believed to be 5000 gal tanks carrying approx. 4000 gal of 28% HCl acid each) and a safety shower trailer were involved and damaged by fire.

Frac crew was pumping stage 16 of 48 on the MOR31F well (125-28269) when the fire broke out.
