Act 9 signage in place. The rig is being moved off-site since 2/23/15; the last well (0H) reached lateral TD on 2/21/15. A panel of the sound wall has been removed to tie-in the gathering pipeline spur and will be replaced within the next few days once line is complete. Fraccing is estimated to take place around mid-March. The site is currently snow covered; however it appears that all E&S controls remain in place and are being maintained. The site appears to be stable and no violations were noted during this inspection.

•Site restoration work including the construction of infiltration berms was being conducted at the time of inspection. •Additional servicing and work was being conducted on the adjacent pipeline connection at the time of inspection. •Vegetation on the cut slope above the pad was sparse and patchy. No violation.

2516658 Primary Facility 2016-04-26 Follow-up Inspection No Violations Noted On 4/26/16, I performed a follow up inspection at the Mad Dog 20/20 site in response to observed conditions noted in a prior inspection on 4/13/16 and made the following observations:

• Temporary stabilization was found in place on all disturbed areas on site. • PCSM control implementation was completed at the time of inspection. • Disturbed areas along the adjacent pipeline were not stabilized at the time of inspection. No violation.

2698777 Primary Facility 2018-02-27 Incident- Response to Accident or Event No Violations Noted On 2/27/18 an incident inspection was conducted at the Mad Dog 2020 Well Site in response to an operator reported slide on the cut slope above the pad. The following was observed:

>>The operator reported substantial ground movement a cut slope of the pad in a location that had multiple previous slides. Site inspection found a large area had slid a small amount with no BMP failure and no impact outside of the site LOD. No violation.

>>The operator requested approval for a minor modification to repair the slide including removing slide material to an on site topsoil pile and replace with coarse stone. All exposed soil to be stabilized and protected with CFS. The modification was approved.

>>The pad was minimally active at the time of inspection and the site was in generally good condition with no noted spill or erosion issues.

>>The complainant reported recent third party testing indicated multiple elevated parameters. Sample 3401-080 was collected from an outdoor tap per standard procedure.
