| On November 8, 2012, Environmental Trainee, Carl Livermore and I stopped at CONSOLï¾’s NV-55 site to conduct a pre-drill inspection of their perimeter E&S controls.
 
The E&S Plans were in the on-site mailbox and I pulled them and walked the site perimeter to check them against what was installed.  For the most part they matched, although I noted that the contractor added silt fence that wasnï¾’t in the original plans.  This is not a problem but it needs to be added to the plans and be initialed by a WQS.
 
CONSOL is using silt sox as their primary perimeter controls; we found some areas that were in danger of being overrun with sediment and some areas that the silt sox was damaged.  Additionally, in some areas the silt fence was not installed properly and in at least one area there was a animal tunnel under the fence.
 
CONSOLï¾’s contractor had the site stabilized via hydroseeding.
 
CONSOL has met pre- operation inspection obligation as outlined under 3258( a)( 1).

| On November 21, 2012, I conducted a follow-up inspection at CONSOLï¾’s NV-55 site.  The site was completely inactive and there wasnï¾’t even anybody in the guard shack.  I checked the E&S BMPs but it did not appear that any maintenance had been performed on them. 
 
I will follow-up with CONSOL with this.

| On July 25, 2013, I conducted an inspection at CONSOLï¾’s Nv-55 site.  At inspection, CONSOL was into the lateral on the 55F well and was at around 11,000 feet.
 
Both the E&S plan and the PPC plan were in the mailbox at the site entrance.  The company needs to update the DEP contacts in the PPC plan.  I also found permits found permits for the A, B, C and F wells; I did not find a permit for the NV-55M well.
 
CONSOL has an Act 9 sign out front. 
 
Containment looked good and the vegetation looked very good although much of the vegetation was annual grasses.
 
I found that there is drag-out from the access road onto the main road; additional BMPs are needed to address this problem.
 
CONSOL is doing daily E&S inspections at the site.
