| On January 13th, 2011 I conducted an inspection of the above site.  A portion of the fill side of the pad had sliped during construction.  I met with Atlas representatives Gary Bowlen, Chad Eisenman, and Nick Staffieri on site.  The fill side was still an estimated 14' below the proposed finished elevation.  Multiple scarp faces were evident with a vetical separation of over 20' at the greatest.  All material that moved stayed within the limit of disturbance.  Some material had entered the interceptor channel at the base of the slope.  No streams or watercourses are in close proximity to the base of the slope.  The material that slid appeared dry and no springs or seeps were observed.  The operator is to install additional E&S controls around the base of the slide and forward the reconstruction plan once designed.

| On January 25th, 2011 I conducted a follow up inspection of the site in response to the slide and inspection on January 13th, 2011.  Additioanl E&S controls were installed on the lower edge at the toe of the slip.  During excavation and reconstruction the material began to move again.  A scarp face of approximately 4' had developed.  Constructed had halted until the engineers can evaluate the soil conditions and formualte a new plan for the excavation.

| On March 10th 2011 I conducted a routine inspection of the above site.  Drilling was completed and the rig had moved offsite.  The drill cuttings were being removed with an excavator and being hauled offsite.  No additional movement was observed on the slide.  The access road was in good condition.  A site ID and E&S plan were posted on site.  Excess runoff from the pad was beginning to crate a ditch and flow around the sediment trap.  Some material upslope of the sediment trap had slid into the trap.  Water flowing around the trap was undermining the filter sock and flowing into a wooded area approx 200¿.  No streams or watercourses close.  Crews were on site immediately to divert water into the sediment trap, a diversion was built at the top of the pad, and repairs and additions were made to the filter sock.

| On June 27th 2011 I conducted a routine inspection of the above site.  Drilling was completed and production units were being installed.  The pit was closed. The slide on the fill slope was not yet repaired awaiting restoration of the site.  The access road was in good condition, Site ID and E&S plan were posted on site.  No erosion was leaving site and was contained within the E&S controls.  The operator should continue to monitor and inspection the E&S controls routinely and after each runoff event until site restoration is complete and permanent stabilization is established.

| On January 13th, 2011 I conducted an inspection of the above site.  A portion of the fill side of the pad had sliped during construction.  I met with Atlas representatives Gary Bowlen, Chad Eisenman, and Nick Staffieri on site.  The fill side was still an estimated 14' below the proposed finished elevation.  Multiple scarp faces were evident with a vetical separation of over 20' at the greatest.  All material that moved stayed within the limit of disturbance.  Some material had entered the interceptor channel at the base of the slope.  No streams or watercourses are in close proximity to the base of the slope.  The material that slid appeared dry and no springs or seeps were observed.  The operator is to install additional E&S controls around the base of the slide and forward the reconstruction plan once designed.

| On March 10th 2011 I conducted a routine inspection of the above site.  Drilling was completed and the rig had moved offsite.  The drill cuttings were being removed with an excavator and being hauled offsite.  No additional movement was observed on the slide.  The access road was in good condition.  A site ID and E&S plan were posted on site.  Excess runoff from the pad was beginning to crate a ditch and flow around the sediment trap.  Some material upslope of the sediment trap had slid into the trap.  Water flowing around the trap was undermining the filter sock and flowing into a wooded area approx 200¿.  No streams or watercourses close.  Crews were on site immediately to divert water into the sediment trap, a diversion was built at the top of the pad, and repairs and additions were made to the filter sock.

| On June 27th 2011 I conducted a routine inspection of the above site.  Drilling was completed and production units were being installed.  The pit was closed. The slide on the fill slope was not yet repaired awaiting restoration of the site.  The access road was in good condition, Site ID and E&S plan were posted on site.  No erosion was leaving site and was contained within the E&S controls.  The operator should continue to monitor and inspection the E&S controls routinely and after each runoff event until site restoration is complete and permanent stabilization is established.

Straw bales had been added behind the filter sock below the sediment trap and repairs were made to the sediment trap. No more erosion was evident. All activities were within the limit of disturbance
1992019 Primary Facility 2011-06-27 Routine/Complete Inspection No Violations Noted On June 27th 2011 I conducted a routine inspection of the above site. Drilling was completed and production units were being installed. The pit was closed. The slide on the fill slope was not yet repaired awaiting restoration of the site. The access road was in good condition, Site ID and E&S plan were posted on site. No erosion was leaving site and was contained within the E&S controls. The operator should continue to monitor and inspection the E&S controls routinely and after each runoff event until site restoration is complete and permanent stabilization is established.
