| On April 21st, 2011 I conducted an  inspection of the above site.  Drilling and frac were complete; production equipment was being installed. Site ID and E&S plan were posted on site. The access road was in good conditon. E&S controls were functioning.  Water coming from the base of the cut slope was entering the E&S interceptor channels.  The water is flowing through the interceptor channels around the perimeter of the site and exiting through the sediment trap.  Evidence of coal spoils is apparent of the cut slope.  The water is staining the rocks in the interceptor channels white and red. Suspected to be acid mine drainage.  Water exiting the sediment trap is flowing over land, through the woods and into bigger run.  The operator was contacted and a meeting was setup for later that week.

Chevron was to begin monitoring the water, flow rates, water chemistry sampling, etc at multiple locations around the pad and keep DEP informed of all progress and findings.

| On April 12, 2012, at about 16:20 Nick Staffieri contacted WQSS Vince Yantko about a minor spill that happened at about 15:20 that day.  This was followed up by a written incident report written by Nick on April 24, 2012.  The review of the incident was assigned to WQS, John Carson who requested and received a copy of the Bill of Landing from Brian Rankin (also of Chevron).

| On May 3, 2012, several representatives from DEP in both the Environmental Clean-Up Program and the Oil and Gas Program met with several representatives of Chevron and their subcontractors at the Robinhill site to get an update of the condensate spill remediation and to discuss where to go from there.

The excavations seem to keep growing and occasionally you could still see condensate on the water in the collection trenches and still smell the condensate in the air.  I took a short movie of the material floating in water in a collection trench.
 
 
We checked the pipeline below the excavations but did not find any condensate there.
 
 
We talked about Act 2 standards and to what standard the site would be restored to.  That is an issue best handled by DEPï¾’s Environmental Clean-up.

The E&S controls were in good shape.  Chevron constructed a sediment pond on the west side of the pad and placed large rock in the effluent end.  Some of the recent rains were very heavy and during these times the operators used a diesel pump to move water toward the north end of the pad where the water could dissipate through the soil.
 
 
 
Much of the excavated soil and fill is gone from the holding area.  A few large boulders remain.
 
 
 
I found no problems during my inspection.

The well pad was constructed, but drilling of the wells has not yet taken place. the pits were also constructed. The access road was in good condition. A site ID sign and E&S plan were on posted site. The E&S controls appeared to be functioning properly and should continue to be inspected routinley and after every significant rainfall event.
1910637 Primary Facility 2010-07-28 Routine/Complete Inspection No Violations Noted On July 28th, 2010 I conducted a routine inspection of the above site. The well was recently spudded. No drill rig was on site at the time of my inspection. A new liner was placed in the pit. The access road was in good condition. A site ID sign and E&S plan were on posted site. The sediment trap at the rear of the pad needed maintenance. Gary Bowlens of Atlas was contacted and a construiction crew was dispatched to the site. The E&S controls appeared to be functioning properly and should continue to be inspected routinely and after every significant rainfall event.
1910643 Primary Facility 2010-08-06 Routine/Complete Inspection No Violations Noted On August 6th, 2010 I conducted a inspection of the above site. The necessary repairs required to the sediment trap noted in the 7/28 inspection had been performed. The sediment trap and interceptor channels were now in good condition. The E&S controls appeared to be functioning properly and should continue to be inspected routinely and after every significant rainfall event.

2005554 Primary Facility 2011-04-21 Routine/Complete Inspection No Violations Noted On April 21st, 2011 I conducted an inspection of the above site. Drilling and frac were complete; production equipment was being installed. Site ID and E&S plan were posted on site. The access road was in good conditon. E&S controls were functioning. Water coming from the base of the cut slope was entering the E&S interceptor channels. The water is flowing through the interceptor channels around the perimeter of the site and exiting through the sediment trap. Evidence of coal spoils is apparent of the cut slope. The water is staining the rocks in the interceptor channels white and red. Suspected to be acid mine drainage. Water exiting the sediment trap is flowing over land, through the woods and into bigger run. The operator was contacted and a meeting was setup for later that week.
2005568 Primary Facility 2011-04-27 Routine/Complete Inspection No Violations Noted On April 27th, 2011 a meeting was held at the above referenced site with representatives from DEP (John Carson, Vince Yantko, Brian Rankin), Chevron (Bob Hirtz, Gary Bowlen, James Braun, Chad Eisman, Nick Staffieri) and their consultant Earthtech (Joe Lechnar) to discuss the AMD drainage issues from the previous inspection (04/21/2011). Water was still flowing from the cut slope and passing through the interceptor channels and sediment trap. Some erosion issues were starting to develop on the gas line below the site as a result of the discharge form the sediment trap. Given the evidience of previous mining activity at the site, it was agreed that Chevron would research the history of the site, including mining activity, preconstruction conditions, and any water quality data. Chevron was to begin monitoring the water, flow rates, water chemistry sampling, etc at multiple locations around the pad and keep DEP informed of all progress and findings.
2005571 Primary Facility 2011-05-27 Routine/Complete Inspection No Violations Noted On May 27th, 2011 a meeting was held at the above referenced site with representatives from DEP (John Carson, Brian Rankin), and Chevron (Chad Eisman, Nick Staffieri) to discuss the AMD drainage issues from the previous inspections (04/21/2011, 04/27/2011). Water was still flowing from the cut slope and passing through the interceptor channels and sediment trap. Ph levels were checked with a field meter to verify results of monitoring being preformed on the site.
2005572 Primary Facility 2011-08-11 Routine/Complete Inspection No Violations Noted On August 11th, 2011 a meeting was held at the above referenced site with representatives from DEP (John Carson, Brian Rankin), DEP Mining Office (Tom Kovalchuk) and Chevron (Chad Eisman, Nick Staffieri, George Bozek) to discuss the AMD drainage. Chevron and their consultant EarthTech had prepared documentation to demonstrate that the discharge was a preexisiting condition as evidenced by historical records and physical evidence of mining activity. The operator is to restore the site and in that process the drainage will be returned to its previous flowpath. DEP mining office was on site to review the preposed plan. Restoration activities are to start immediately. The operator is to submit a final plan that will address the drainage and site restoration. Site montioring is to continue to observe restoration success.

This was followed up by a written incident report written by Nick on April 24, 2012. The review of the incident was assigned to WQS, John Carson who requested and received a copy of the Bill of Landing from Brian Rankin (also of Chevron).
