2593564 Primary Facility 2017-05-11 Routine/Complete Inspection No Violations Noted On 5/11/17 at 1313 hrs I conducted a routine inspection at the Kennedy 137 well site. It was overcast with showers and 50 degrees F. SWEPI On-Site Representative (OSR), Jake Huggard, was present.

The 1H – 7H and 9H wells have been drilled. The rig and other drilling equipment has been demobilized and removed from the site. Several contractors were present; Mr. Huggard stated that operations at the time of my inspection included cement bond logging of the wells and nippling up on the tubing spools in preparation for the frac.

The pad surface was somewhat muddy. The secondary containment used for the rig remained in place, however, the perimeter was unbermed in areas. It is recommended that the containment integrity be evaluated prior to the frac commencing. Any breaches should be repaired and the perimeter re-bermed.

E&S controls and site stabilization were observed. The slump in the northeast corner cut slope of the well pad was still present. It was previously noted in the 1/9/17 inspection report. The

2618064 Primary Facility 2017-07-20 Routine/Complete Inspection No Violations Noted I met Dean Albright, SWEPI company man when I arrived. Workover rig was present on the site. I noted water in the eastern drainage ditch, which was directly behind the Baker tank battery containment; conductivity reading in this fluid was 9.11 mS/cm. I proceeded to test soil conductance in the moist soil of the ditch to the north and south of this fluid. To the north, soil conductance readings were between 1.2 and 2.2 mS/cm until approximately 25-30 feet from the fluid, at which point it dropped to .5 mS/cm. To the south, readings were between 1.2 and 2.1 until approximately 80-90 feet from the fluid. I took a sample of the fluid (#0939052) at 09:55 hrs. On the pad itself, I noted salt staining on the soil between the workover rig containment and the Baker tank battery. Soil conductance reading in this area was 9.44 mS/cm. I noted an area of at least 8' x 8' that showed evidence of salt staining, and the affected area could be much larger. Dean accompanied me as I collected the sample and checked t
2618940 Primary Facility 2017-07-25 Routine/Partial Inspection No Violations Noted I met with Pat Bernethy and Dean Albright on the site to discuss my findings from my 7/20/17 inspection, as well as what SWEPI has done since. The workover rig was still on location. We looked at the area I discovered salt staining to the northeast of the workover rig; a large puddle (15-25 gallons) was present in this area. Conductance readings in this puddle were from 3.3-4.0 mS/cm. Pat asked if they could potentially wait until the workover rig finished drilling out plugs and the containment was removed to clean this area (estimated to be around August 15th); I recommended that they attempt to scrape this area as soon as practicable. The ditch where I discovered the fluid with elevated conductance had been excavated on 7/21/17, and was screened with a soil conductance meter to determine the extent of the excavation. The depth of the excavation appeared to be less than 1', and ran from 50-60' north of the location I took my sample to the rock lined ditch that drains into this ditch at 80-90' south of
2626340 Primary Facility 2017-08-15 Administrative/File Review No Violations Noted Admin inspection is a follow-up to pending violations on the site. Original inspection revealed salt staining on the pad surface and fluid with elevated conductance in the drainage ditch. Sample ID #0939052 collected from ditch; chemical analysis shows elevated barium (189700 UG/L), strontium (178300 UG/ L), and chlorides (4490 MG/L). This indicates a release of a pollutional substance to the pad and ditch. Violations assigned to permit number 117-20207.

2654726 Primary Facility 2017-10-31 Routine/Complete Inspection No Violations Noted A routine complete inspection was conducted at the SWEPI LP Kennedy 137 well pad located in Delmar Township, Tioga Co. on October 31, 2017 at 1300 hrs. The weather was 42 degrees and cloudy, winds WSW 14 mph. Chris Underwood, OSR was Shell's onsite representative.

The Kennedy 137D well is currently on "regulatory inactive status". SWEPI is in the final stages of flowing back the 1H, 2H, 3H, 4H, 5H, 6H, 7H and 9 H wells. The flow back setup consist of sand traps, a gas buster, separator units and the highland tanks. Produced fluids from the Highland tanks are being pumped to the baffled tanks located on the east side of the pad. These fluids are then being trucked, as needed, to the Lingle 1102 pad for reuse in the frac.

Numerous puddles were observed on the pad surface and were screened for field conductivity. None of the puddles showed elevated conductivity. Secondary containment holding water was also screen for field conductivity. No elevated readings were detected.

The weather was 85 degrees, partly cloudy, 71% humidity and winds 7 mph ssw. The site is set up for flowback/production currently and no company personnel were onsite at time or inspection. General inspection of wellheads, temporary storage, E&S controls, production tanks and reclaimed pony pad were all conducted.

No violations noted at this time. We departed the site at 1500 hours.

2832283 Primary Facility 2019-01-28 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Kennedy 137 Well Pad in Delmar Twp, Tioga County on 1/28/2019 at 1140 hours. The weather was sunny, 14 degrees and the pad was snow covered where not plowed.

Proper act 9 signage was located at the start of the access road. The onsite inspection was log and E&S Plan was found inside the mailbox behind the sign. The last inspection was conducted by company personnel on 1/12/2019 and noted no issues. There was a water truck onsite offloading water from the highland production tanks onsite. The pad was mostly ice covered. PCSM BMPs were observed functional but snow covered. There are currently 9 wells onsite capable of production. All fluids on the pad surface were frozen solid at time of inspection. No spills or releases were observed onsite.

No violations noted today, I departed the site at 1220.
