2569714 | Primary Facility | 2017-03-09 | Routine/Complete Inspection | No Violations Noted | Inspection conducted to assess site conditions after drilling operations. As noted in the previous inspection, temporary E&S control BMPs are in place and no evidence of accelerated erosion or sedimentation was observed. No evidence of any spills, such as odors, sheens, or stains was observed on the pad surface. No violations were cited during this inspection. |

2717011 | Primary Facility | 2018-04-11 | Routine/Complete Inspection | No Violations Noted | Weather is 33° and snowing at time of inspection. Met with SWN representative Jonathan Woodburn on location. Rig was in the process of drilling the 17 1/2" hole for the surface casing at time of inspection. Estimated TD for the surface hole is 758'. They are estimating cementing casing in the early AM on 4/12/18. No violations noted at time of inspection. |
2718238 | Primary Facility | 2018-04-12 | Routine/Complete Inspection | No Violations Noted | Weather was 48° and raining at time of inspection. Met with SWN representative Jonathan Woodburn on location at time of inspection. Rig was in the process of running 13 3/8" surface casing into the hole. Halliburton was on location to perform the cement job for the surface casing. Cement job appeared to be going well when I departed location. Plan to follow up on the cement job early next week. No violations were noted at time of inspection. |
2718936 | Primary Facility | 2018-04-17 | Routine/Complete Inspection | No Violations Noted | Weather was 33° and cloudy at time of inspection. Met with SWN representative Jonathan Woodburn on location. Rig was in the process of drilling then 8 3/4" vertical section of the production hole on air. They plan to switch over to oil based mud at around 6400'. They plan to kick off at approximately 6173' ft. Reviewed the job log from the intermediate cement job and no discrepancies were noted. No violations were noted at time of inspection. |
2722007 | Primary Facility | 2018-04-24 | Routine/Complete Inspection | No Violations Noted | Weather was 52° and cloudy at time of inspection. Met with SWN representative Jonathan Woodburn on location. Rig was in the process of tripping drill pipe out of the hole. Well was TD'd at 14861' at approximately 11:30 on 4/23/2018. Rig is anticipating beginning running casing later this evening and cementing Wednesday night. No violations were noted at time of inspection. |
2723304 | Primary Facility | 2018-04-26 | Routine/Complete Inspection | No Violations Noted | Weather was 52° and cloudy at time of inspection. Met with SWN representative Andrew Griffith on location. Rig was in the process of waiting on cement and preparing to rig down to move to another location. Halliburton bumped the plug on the production casing at 10:21 pm on 4/25/2018. Reviewed the cement job log and no discrepancies were noted at time of inspection. No violations were noted at time of inspection. |
2724862 | Primary Facility | 2018-05-01 | Routine/Complete Inspection | No Violations Noted | Weather was 61° and sunny at time of inspection. Well was recently drilled and cemented by the SDC 45 right. The well was shut in and capped at time of inspection. There were analog pressure gauges attached to the 5 1/2 x 9 5/8 anulus and to the 5 1/2 inch casing. The gauges were all shut in and read zero psi at time of inspection. No violations noted at time of inspection. |
2725577 | Primary Facility | 2018-05-01 | Routine/Complete Inspection | No Violations Noted | No activity on the pad when I arrived. Pipeline work at entrance and across the road. Containment was partially dismantled, with some of the rig mats removed and stacked, as well as some berms removed and stacked. Puddles on the well pad showed low conductance (.9 -1.6 mS/cm). The 8H well cellar was nearly full to the top with water; I recommend that it be vacced ASAP. Site looked good overall. |
2738661 | Primary Facility | 2018-06-05 | Routine/Complete Inspection | No Violations Noted | I met Will Stevens of SWN in the command trailer when I arrived. Frac was active (Halliburton), although it was pressured down for a move from the 7H to the 8H well. I was joined on my inspection by Jeremy Eichenberger and Allen Green of SWN. The site and sitewide containment were very clean, with no standing water. I did note in 3 locations (near the sand boxes, near the production tanks, and near the GPUs) where the sitewide liner had fallen or been blown off of the Jersey barriers forming the berms, and pointed these out to Jeremy and Allen. The issue seemed to be that the liner had not been installed long enough to reach the ground on the backside of the Jersey barriers; Jeremy and Allen proposed seaming an additional piece of liner to the existing liner to allow the liner to reach the ground so it could be secured with sandbags to keep it in place. |
2773337 | Primary Facility | 2018-09-04 | Routine/Complete Inspection | No Violations Noted | Southwestern Energy (SWN) submitted a well site restoration report to the Department for SWN TI-14-Connolly A Pad on 8/22/18.
Department staff Mike Jurbala, Sara Metz, and Sasha Minium conducted a site restoration inspection at the above site on 9/4/18. Two SWN employees remained in their vehicles during the inspection. There are 3 Marcellus wells staged for production on the pad (Connolly A 2H, 7H, &8H). The wells were in secondary containment and the well cellars were covered by the containment. There are 3 GPUs and 1 production water tank within secondary containment. The site meets 70% uniform, perennial, vegetative cover. All PCSMs have been installed as depicted on PCSM/SR plans. However, at the time of inspection flowback water tanks remained on site. As per 78a.65(a)(1) these tanks, their associated secondary containment, the secondary containment around the wells and any other equipment or containment not needed for production should be removed as part of the site restoration plan. 2811141 | Primary Facility | 2018-12-06 | Routine/Complete Inspection | No Violations Noted | Weather was 30° and cloudy at time of inspection. There were no Southwestern representatives on location at time of inspection. Well had digital pressure monitoring equipment on the production tubing. Production tubing pressure was not recorded due to condensation in the gauge at time of inspection. There was one annulus vent plumbed to surface. It was plumbed into the pay line. The Surface and intermediate casing annuli were visible at time of inspection. 0% combustible gas was detected in any area surrounding the wellhead at time of inspection. No violations were noted at time of inspection. |
