| Preoperation Inspection

| Casey Baldwin and I performed a preop inspection on the Moore S pad.  The perimeter BMPs were all in place and operational, despite recent torrential rains.  The pad was under construction at the time of inspection.  The RCE was in place as was the entrance culvert.  The tank pad was not constructed yet however the perimeter BMPs were all installed.  I approve this preoperational inspection.  The wells associated with this site are the 115-22392- 115-22393, 115-22394 and 115-22395.

There were no violations.

| At time of inspection.

Met with company man Larry Cheatwood.

United Drilling was on site to drill and run the conductor pipe for the 2H, 4H, 6H and 8H wells.

The 2H was at TD at 103'.

There was an auger rig on the 4H at a depth of 20'.

The 6H was at a depth of 46'.

There was an air rig on the 8H at a depth of 84'.

The TD for all the wells will be 103'.

I took a walk around the location and there were no obvious concerns noticed.

There were no violations noted.

Nabors X36 rig was on site and drilling the lateral section of the well.

There are four wells on site to be drilled, the surface and intermediate casings have been drilled and set on all four wells.

They are drilling the hole for the production casing on the second well out of the four.

They are drilling with DOBM.

The proposed TD for the well is 19744'.

Keane was on site to frac the 2H, 4H, 6H and 8H wells.

They were stimulating stage 32 out of 66 stages on the 8H and running a plug and perf in the 4H when I arrived on site.

There were 23 stages completed out of 59 on the 2H.

There were 29 stages completed out of 67 on the 4H.

There were 32 stages completed out of 70 on the 6H.

The water was being trucked to the site.

The well was recently completed and put in production.

The well tag was on the well and correct.

There was water 7.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar.

The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank.

There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector  that was calibrated on 11/2/18.

The production fluid was going to a 500bbl tank on containment and the containment appeared to be in good condition.

There were 16 empty 500bbl tanks that were used during the frac, 12 of those tanks were cleaned and not on containment and the other 4 tanks were on containment and the containment appeared to be in good condition.

The 6H was at a depth of 46'.
 +
There was an air rig on the 8H at a depth of 84'.
 +
The TD for all the wells will be 103'.
 +
I took a walk around the location and there were no obvious concerns noticed.

The well was recently completed and put in production.
 +
The well tag was on the well and correct.
 +
There was water 7.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar.
 +
The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank.
 +
There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector  that was calibrated on 11/2/18.
 +
The production fluid was going to a 500bbl tank on containment and the containment appeared to be in good condition.
 +
There were 16 empty 500bbl tanks that were used during the frac, 12 of those tanks were cleaned and not on containment and the other 4 tanks were on containment and the containment appeared to be in good condition.
