I met with company man Glenn Broussard. United Drilling was on site to drill and set the conductor pipe. There was one auger rig and two air rigs running. The auger rig finished running the 26" pipe to 37' on the 16H and moved over to the 14H and started drilling. One air rig was on the 9H at 30' and the other air rig TD at 94' on the 15H and was pulling drill pipe. The 8H, 12H, 17H, and 18H were completed with the conductor pipe ran and cemented at 94'. I took a walk around the location and there were no obvious concerns noticed. I left the site at 10:55a.m.

2622713 Primary Facility 2017-08-03 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 9:10a.m.

I met with company man Perk McDowell. Nabors X36 Rig was on site and preparing for Schlumberger to cement the surface casing. The rig will wait until the cement is at 500psi CS (Compressive Strength) which is 5 hours and 38 minutes before putting the BOP on the well head. They bumped the plug at 11:11a.m. I took a walk around the location and there were no obvious concerns noticed. Depth at time of inspection was 623'. I left the site at 12:40p.m.

2623668 Primary Facility 2017-08-09 Routine/Complete Inspection No Violations Noted Today's inspection, 7/19/17 is being conducted as a routine inspection. No pictures were added to this inspect due to an electronic glitch. Drilling rig still on-site. Signage is up and displayed. Good vegetative growth occurring on all sections of the site. Vegetation a mixture of annuals and perennials with clovers being the most dominant perennial. No violations were observed during this inspection. Drilling set up looked very streamlined and organized.
2624141 Primary Facility 2017-08-07 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 12:40p.m.

I met with company man Perk McDowell. Nabors X36 Rig was on site and drilling the hole for the intermediate casing on the 17H well. I took a walk around the location and there were no obvious concerns noticed. Depth at time of inspection was 931'. I left the site at 14:10p.m.

2627629 Primary Facility 2017-08-17 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:00a.m. as part of a migration investigation, accompanied by DEP Geologist Andrea Mullen and WQS Gene Rickard.

We met with Steve Watson and Marshall McCarty of Cabot Oil and Gas and Prad Jadeja of Selman & Associates. Andrea used an Altair 5X gas meter calibrated on 8/16/17 and Prad used a Sensit HXG-3 gas meter to check for gas. We checked for gas at the vent lines at the edge of the pad, if gas was detected at the (13x9) annulus vent line, we went to the cellar so we could determine if the gas was coming from the (20x13) annulus or the (13x9) annulus. The 9H, 7H, 8H, 10H, 14H, and 12H wells were drilled to TD. The 11H, 15H, 13H, 17H, 16H, and 18H wells were partially drilled with the surface and intermediate casings cemented into place I left the site at 13:30p.m.

2639070 Primary Facility 2017-08-17 Complaint Inspection No Violations Noted Todays inspection, 8/17/17 was conducted as part of a Department investigation into a nearby water supply complaint. This inspection centered around investigating the annuli of the 12 wells recently drilled at this location. Present during this inspection were myself, Jon Sassi, Dept EPS, David Schoonover, Dept OGI, Steve Watson, Cabot EH&S Specialist, Marshall McCarty, Cabot Production Person and Prad Jadeja, Cabot consultant. As part of the effort in trying to identify if any of the wells are discharging gas from their respective annular vents, Cabot has developed a more sophisticated layout of the vent pipes which are clearly marked. No E&S violations were noted.
2663272 Primary Facility 2017-11-21 Drilling/Alteration No Violations Noted At time of inspection.

Met with company man Perk McDowell. Nabors X36 rig was on site to drill the lateral section of the well. The proposed depth of the well is 18,979. The rig was running back in the hole with the drill pipe at the time I arrived on site. I took a walk around the well and there were no obvious concerns noticed. There were no violations noted.

2664132 Primary Facility 2017-11-20 Drilling/Alteration No Violations Noted I arrived at the site at 12:24. The weather conditions are mostly cloudy skies, with a light breeze and occasional wind gusts, with temperatures in the mid to upper 30's. Nabors X 36 is the drilling rig on-site. Cabot submitted a Drilling resumption After Break of 30 days or more notification to the Department on 11/15/17 for the Jeffers Farms Pad 2 18H well. The notice indicates the date and time the action was to occur was on 11/16/17 at 6:00 AM. I met with Perk McDowell, site company man, consultant of Cabot. Mr. McDowell informed me of the following regarding site activities. "We are drilling the 18H well on diesel oil based mud at a depth of (6,954) feet. We are drilling below the kickoff point in the curve. Kickoff point (KOP) was at (4,843) feet. Curve will be landed at (8,575) feet. The well will be drilled to total depth (TD) at (18,979) feet. Drilling resumed here on 11/18/17. Surface and intermediate casings have been cemented. The well was drilled to KOP and then the rig left the pad. We resumed
2719468 Primary Facility 2018-04-17 Routine/Complete Inspection No Violations Noted At time of inspection.

Met with company man Scott Barnhart. Keane was on site to frac the 8H, 10H, 12H, 14H, 16H and 18H wells. They were on stage 32 out of 46 stages on the 12H and running the plug and perf in the 18H at time of inspection There were 23 stages completed out of 35 stages on the 8H. There were 24 stages completed out of 36 stages on the 10H. There were 25 stages completed out of 37 stages on the 14H. There were 46 stages completed out of 52 stages on the 16H. There were 47 stages completed out of 55 stages on the 18H. I took a walk around the location and there were no obvious concerns noticed.

2730567 Primary Facility 2018-05-15 Routine/Complete Inspection No Violations Noted I arrived on site at 1345 to perform a RTNC inspection of the Jeffers Farms P2 well pad. Present during the inspection were Casey Baldwin (WQS - DEP) and Ken Wilson (EHS Foreman - Cabot).

The entrance to the access road was well-marked by the appropriate signage. The access road was stable and solid with no apparent erosion or deterioration. The well's mailbox contained most of the required paperwork, however the site map outlining the BMPs associated with the ESCGP permit was absent. I used the site map with PCSM associated BMPs instead.

On the date of inspection, the wells on the pad were in between stages of hydraulic fracturing. Fracturing had been completed on the 8H, 10H, 12H, 14H, 16H, and 18H. Hydraulic fracturing had yet to be performed on the 7H, 9H, 11H, 13H, 15H, and 17H.

Several nearby water supply complaints have prompted Cabot to install a monitoring station on site to aid in early detection of any further issues that may arise.

2762359 Primary Facility 2018-08-07 Follow-up Inspection Pending At time of inspection.

The wells were recently completed and put in production. Cabot Oil & Gas contacted the Department to let them know they plan to evaluate and take isotopic samples from the annuli of the Jeffers Farms P2. The Department Eric Rooney (Geologist) and David Schoonover (Oil & Gas inspector) was on-site to observe and document results. We met with Cabot representatives Mike Hall and Ben Whitaker and Selman representatives Tyler Thomas and Kaydee Oliver. Remedial work was done on the 14H, 8H, 7H and 10H wells. Work started in August 2017 and well assessments are still ongoing.

18H 115-22261 All annuli were connected together and venting to a poly vent tank. (9x5) 44% LEL 22000ppm sample taken (13x9) 20% LEL 10000ppm sample taken (20x13) 30% LEL 16200ppm sample taken Since some presence of gas was detected at all wells annuli, it appears that some comingling of gas may have been occurring within the current vent plumbing set up.

2855468 Primary Facility 2019-03-13 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. Overall the site appears to have good stabilization. As Photo #2 #3 show, a sheen was noted on the containment which is still present from the stimulation activities which occurred at this site. Please vac this containment and remove immediately. Steve Watson, Cabot EH&S Specialist was notified about this sheen during the inspection. No E&S issues identified. No violations noted.
2856414 Primary Facility 2019-03-20 Follow-up Inspection No Violations Noted Today's inspection was conducted as a follow-up inspection to the Department's inspection conducted on 3/13/19 in which an oily sheen was noted on standing water which was on containment present on this pad. Today's inspection finds that the contaminated water was removed, the 3 mobile water tanks have been removed, and the containment is being removed. Standing water thus far exposed after containment removal has no sheen present. No violations noted.
