2524908 Primary Facility 2016-09-15 Routine/Complete Inspection No Violations Noted On 9/15/16, the Department conducted an inspection of the Jeffers Farms Pad 5 located in Gibson Township, Susquehanna County in response to Cabot’s self-reporting of a small spill of approximately 20 to 30 gallons of hydraulic fluid to the tank pad surface caused when a hydraulic line on a dump truck failed. Upon arriving at the site, a Resource Environmental Technician was on-site overseeing the remediation of the spill. The technician told me that they were revising their estimate of the spill down to 5 gallons based on the fact that when the hose was repaired on the dump truck, only 5 gallons of hydraulic fluid was added to the trucks hydraulic fluid reservoir. A review of what had been already excavated and what needed to be excavated suggests the 5 gallon estimate to be accurate. The Resource technician stated that they planned on using a PID to confirm their visual assessment of a successful remediation. A quick review of the overall pad found that all E&S controls to be in place and installed to
2529058 Primary Facility 2016-10-06 Preoperation Inspection No Violations Noted On 10/6/16, the Department was on site at the COG Jeffers Farms P5 Well Pad to conduct a Preoperational Inspection. The wells associated with this pad are the Jeffers Farms 50H, 51H, 52H, 53H, 55H, 57H, 59H, 61H, and the 63H. During the inspection, a cross culvert with an associated level spreader was identified near the entrance, but was not listed in the original site drawings. The culvert has been redlined onto the site plans as indicated in Photograph 1. The road to the well pads is a Township road (Jeffery Road T-507). The existing cross culvert (shown on Drawing ESCPC-02) was a 15” CMP that was removed and replaced with a 15” HDPE pipe during resurfacing of the road as required by the Township. The approval for the installation of the pipe was not requested as it already existed on the Township road and was required by the Township to be reinstalled. 2 large top soil stock piles have been added to the site drawings, 1 is between the well pad and the tank pad, while the 2nd one is just off of the w
2544333 Primary Facility 2016-12-12 Routine/Complete Inspection No Violations Noted On 12/12/16, the Department conducted an inspection of the Jeffers Farms Pad 5 located in Gibson Township, Susquehanna County in response to Cabot’s self-reporting of a small spill of approximately 10 gallons of compressor oil to the well pad surface caused when a valve on the compressor of one of the spudder rigs on-site was left open. When the compressor was fired up, the crew noted the release and shut down immediately. Upon arriving at the site, I met with Steve Watson, Cabot Senior EH&S Specialist. A Resource Environmental Technician was on-site overseeing the remediation of the spill. The Resource technician was utilizing a PID to evaluate and confirm remediation success. Approximately 20 yards of pad material has been removed thus far, with readings suggesting remediation is almost complete. During this inspection, another small spill occurred on the pad surface. A skid steer blew a hydraulic line near the well heads. Approximately 20 gallons of hydraulic fluid was released to the ground. Mr.
2545896 Primary Facility 2016-12-19 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 8:45a.m. for a new spud inspection, I met with company man James Cheek. United Drilling was on site to drill the holes and run the conductor pipe. The rig was drilling the mouse hole for the 52H. The conductor pipe was drilled and set at a depth of 116' for the 61H. I took a walk around the location and there were no obvious concerns noticed.

2552395 Primary Facility 2017-01-13 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 8:35a.m., I met with company man Joel Franks, Precision Rig 548 was on site and rigged up over the 63H and the surface casing. Halliburton was on site and setting up to cement the casing. I took a walk around the location and there were no obvious concerns noticed.

Depth at time of inspection was 816'. I left the site at 10:00a.m.

2562378 Primary Facility 2017-02-16 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 8:40a.m., I met with company man Joel Franks, Precision Rig 548 was on site and drilling the lateral section of the well. I took a walk around the location and there were no obvious concerns noticed.

Depth at time of inspection was 12561'. Projected TD for the well is 15458'. I left the site at 9:40a.m.

2578707 Primary Facility 2017-03-29 Routine/Complete Inspection No Violations Noted Inspection conducted with Mike Pirner on 3/9/17 of the COG Jeffers Farms P5 Pad located in Gibson Township, Susquehanna County. Todays inspection is Cabots first communication to the Department that they would like to modify the access road in such a manor as would satisfy various entities ability to access properties adjacent to the Well site. This first field survey conducted by the Department did not reveal any obvious reason not to move forward with submitting proposed changes to the Department for review. Drilling rig actively drilling during inspection. No violations observed.
2616370 Primary Facility 2017-07-18 Routine/Complete Inspection No Violations Noted Resource environmental was on site overseeing the clean up. Field screening has been completed with all indications cleanup complete. 2 roll off containers of material have been placed in 2 covered roll off containers. 25 to 30 cubic yards of material has been collected. Overall conditions suggest the 30 gallon figure to be accurate. The Department concurred with Resource that the spill had been adequately cleaned up. No other violations were noted.
2635152 Primary Facility 2017-09-12 Routine/Complete Inspection No Violations Noted At time of inspection.

Met with company men Larry Fulmer and Ken Wilson. Keane was on site to frac the 50H, 51H, 52H, 53H, 55H, 57H, 59H, 61H, and 63H wells. They were on stage 6 out of 67 stages on the 52H well. There was 14 stages completed out of 63 on the 50H well. There was 3 stages completed out of 28 stages on the 51H well. There was 26 stages completed out of 28 stages on the 53H well. There was 26 stages completed out of 27 stages on the 55H well. There was 45 stages completed out of 48 stages on the 57H well. There was 45 stages completed out of 52 stages on the 59H well. The 61H well was completed with 55 stages and the 63H well was completed with53 stages. The water is being hauled in to the tank farm and pumped to the frac site. I took a walk around the location and tank farm and there were no obvious concerns noticed. There was no violation noted.

2669837 Primary Facility 2017-12-11 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put into production. The well tag was on the well and correct. The (20x13), (13x9) and (9x5) annulus vent pipes were connected together and venting to the same tank. There was a 500bbl flow back tank on containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2773250 Primary Facility 2018-09-11 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 6.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2839815 Primary Facility 2019-02-13 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. As the photo's show, the site is snow covered. Overall the site appears to have good stabilization. No E&S issues identified. No violations noted.
