There was 772psi on the production casing and 398psi on the production tubing. I took a walk around the location and everything looked OK.

I left the site at 11:50a.m.

2540572 Primary Facility 2016-11-28 Incident- Response to Accident or Event No Violations Noted The Department (Jon Sassi and I ) were on site in response to a self reported release of glycol. On 11/19/16 at 1524 hours the Department recieved the report that 2 gallons of antifreeze were released to containment from a glycol bath on the gas production unit.

On site the Department noted that the E&S sign in sheet stated that Phil Kithcart conducted an inspection on 11/19/16. Also, the Department reviewed the E&S Plan Map and found that all of the filter sock was redlined from the plan and noted to have been removed.

The infiltration basin #1 was noted to not have appropriate vegetation at the base. This area should be maintained per your E&S Plan.

The Department inspected the area around the gas production unit where the release occurred. The Department screened the fluid within the containment and found that no readings appeared to be elevated. Measurements ranged from 0.02-0.05 milliSiemens. No visual evidence of the release impacting the ground outside of containment was found.

2589732 Primary Facility 2017-05-03 Routine/Complete Inspection No Violations Noted The site was stable and the vegetation was greening up for spring nicely. There was no erosion currently occurring and there was no bubbling in the well cellars. The access road was a bit lumpy but had no stability issues. The pad surface was for the most part level and graded well.

2591610 Primary Facility 2017-05-05 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:15p.m.

The cellar was full of water with no bubbling noticed. The (20x13) annulus vent pipe was open and venting into the atmosphere with no gas detected, the (13x9) annulus vent pipe was open and venting into the atmosphere with no gas detected, the (9x5) annulus vent pipe was open and venting into the atmosphere with no gas detected. There was 478psi on the production casing and 367psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:40p.m.

2699171 Primary Facility 2018-02-27 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water .5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2736948 Primary Facility 2018-05-30 Routine/Complete Inspection No Violations Noted The Department arrived on site at 1235 hrs to conduct a RTNC inspection. The Department was represented by Frank Hoehle (DEP-WQS), John Sassi (EPS-WQS) and myself. Upon arriving at the site, we met with Repsol's Company Man/Completions Consultant Brad Campbell.

Mr. Campbell informed us that eight new wells were being drilled on the pad. All eight conductors had been spudded and drilling was planned to begin the next week. Containment (100 Mil New Pig liner) had been laid down on most of the pad and wooden/rubber mats had been installed over much of the containment. Several frac tanks were already present on the pad to store the oil-based drilling mud that would be needed to perform drilling operations in the following week.

The pad also contained two wells currently in production: the 5H and 10H. However, they were slated to be shut in the next morning because the drilling crew needed the space currently occupied by the production fluid tanks associated with these two producing wells. Yellow cages were sta

2778066 Primary Facility 2018-08-29 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 12:18 in response to a reported spill of oil based drilling mud to the pad surface as a result of compromised secondary containment liner integrity. The weather conditions are mostly sunny, skies, breezy, very warm and humid with temperatures in the mid 80's. Kris Perritt, Environmental Health and Safety Specialist of Repsol contacted me on 8/23/18 at 13:42 to inform me of staining discovered in the general area of the pad surface where the secondary containment liner used to stage frac tanks storing oil based drilling mud was removed. I contacted him during the inspection to inquire of the specifics of the spill incident and the approximate location. He informed me that (2) barrels of oil based drilling mud was spilled to the containment during drilling of the Caprio wells, with (2) gallons of the mud escaping containment due to compromised liner integrity. He further stated that the spill has been cleaned up and the impacted material has been transported to a landfill for disp
2807551 Primary Facility 2018-11-15 Routine/Complete Inspection No Violations Noted I arrived at the site at 9:41 AM. The weather conditions are cloudy skies, cold, with a light breeze and temperatures in the 20's. Repsol is rigging up for hydraulic fracturing (stimulation) of the Caprio 1H-4H, and 6H-9H wells. The Caprio 5H and 10H wells are the existing wells on the location drilled to total depth and stimulated. An employee from Allied Horizontal stated that Repsol is in the process of toe prep stages of the frac, which consists of popping the sleeve, and perforating the first zone in the bottom of the well. Just prior to departing from the pad, I met with Marc Bryant, Field Supervisor of FDS. I had inquired with Mr. Bryant in reference to a reported spill of (1) gallon of hydraulic fluid spilled to the ground at 472 Abell Road, Little Meadows, PA on 11/12/18. This address is in a relatively close proximity to the Repsol Caprio Pad, and the Repsol Abell Pad, which is located in Warren Township, Bradford County. 2820106 Primary Facility 2018-12-11 Incident- Response to Accident or Event No Violations Noted Jared Jacobini, DEP O&G WQS, out of PADEP NCRO in Williamsport, PA and I arrived at the site at 12:55 in response to (2) separate reported spill incidents. The weather conditions are partly cloudy skies with light to moderate winds, and temps in the 30's. Pro Frac is the Repsol subcontractor in the process of conducting hydraulic fracturing (stimulation) operations. Repsol submitted a stimulation notification to the Department on 11/26/18. The comments in the notification indicated frac operations would commence on 11/28/18 at 6 AM. We met with Ralph Babcock, site safety man of REM Security, and Billy Watts, site company man, and completions consultant of Repsol. Zipper frac operations are in progress according to Repsol for the Caprio 1H, 2H, 3H, 4H, 6H, 7H, 8H, and 9H wells. The 5H and 10H wells which were in production, but are currently shut in. Mr. Watts stated the following in reference to the spill incidents. "The spill of (1) quart of motor oil occurred on 12-5-18. The oil leaked from a sight
2826205 Primary Facility 2019-01-02 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 12:13 in response to (3) small spill incidents that recently occurred at the site. The weather conditions are cloudy skies with calm winds, and temperatures in the low to mid 30's. I met with Billy Watts, site company man, and completions foreman of Repsol, and Dave Coxhead, site safety man of REM security. Pro Frac is the Repsol subcontractor at the site in the process of conducting hydraulic fracturing (stimulation) operations. Mr. Watts provided me with the following details in reference to site activities and the spill incidents. "Well swap and zipper frac are in progress. The Frac is on the Caprio 8H well on stage 9. Frac is conducted during daylight hours only. The fire suppressant spill happened at 12:45 PM on 12-29-18. Fire suppressant went off while Pro Frac was in the process of testing the system. 2838540 Primary Facility 2019-02-06 Follow-up Inspection No Violations Noted The Department, Frank Hoehle, DEP O&G WQS and I arrived at the site at 11:39 to conduct a follow up inspection in reference to a production fluid spill that occurred at the site on 1-6-19. The weather conditions are cloudy skies, breezy, with rainfall approaching, and a temperature of 35 degrees Fahrenheit. For additional details pertaining to the production fluid spill, refer to the Department's 1-6-19, 1-9-19, and 1-17-19 inspection reports, Inspection ID#'s 2831500, 2832214, 2832221. We met with Murray Kush, site company man, completions consultant of Repsol, and Dave Coxhead, Site Safety Man, of REM Security. Deep Well Services Rig # 2 is in the process of drilling out hydraulic fracturing plugs. Mr. Kush informed the Department the following. "The rig is drilling out frac plugs on the 3H well. A total of 22 plugs will be drilled out. We are the 20th plug. The 1H is done. Frac plugs will be drilled out on the 7H next. The working tanks and gas buster tanks near the pad entrance contain fluid
