2214965 Primary Facility 2013-10-16 Incident- Response to Accident or Event Pending Arrived at the site at 10:15 in response to a (42) gallon spill of brine that occurred on 4/28/13. The Marcellus incident report indicates the spill occurred at 6:00 PM on 4/28/13 from the top of a hatch of a (500) barrel frac tank and had been cleaned up during the phone call discussion between Mr. Webster and NERO ERT. Mr. Webster indicated the actual material spilled was untreated flowback fluid. Cabot was in the process of well flowback procedures when the spill occurred. The spill occurred as a vacuum truck was loading the raw flowback fluid into a frac tank in the Southwest section of Well Pad. The truck driver did not check the fluid level in the tank prior to off-loading. The tank overflowed and the containment system in place did not have enough volume to hold the flowback fluid. The fluid impacted the surface of the ground in this general area of the Well Pad.
2291831 Primary Facility 2014-07-23 Incident- Response to Accident or Event Violation(s) Noted Steve Watson of Cabot contacted the PADEP Northeast Regional Office Emergency Response Team at 18:12 on 7/22/14 to a report a spill of (42) gallons of glycol. Mr. Watson indicated during the phone call, that due to a compromised check valve approximately (42) gallons of glycol spilled onto a concrete pad and approximately (5) gallons spilled onto dirt. Mr. Watson stated that the spill occurred due to a dump line washing out on the L Aldrich 1H Gas Production Unit. Due to excessive pressure or sand, the wash out of the dump line created a hole in the glycol bath. Glycol was released out of the hole inside the GPU eventually escaping the unit and impacting the concrete containment for the L1H GPU and 2H GPU and the ground surface of the well pad next to the units. Staining and pools of glycol were observed inside and outside of the units and in an excavation (1) to (1.5) feet below the pad surface next to the concrete containment of the 1H GPU. The spill is now estimated to be between (3-5) barrels, with 2-4) barrels impacting pad surface,
2341029 Primary Facility 2015-02-03 Incident- Response to Accident or Event No Violations Noted On 2/3/15, the Department received a complaint that hydraulic fracturing activities occurring on the Cabot Aldrich L. Pad had caused damage to the complainant’s house via causing a possible mini earthquake. The Department confirmed administratively and through field observations that Cabot is in fact conducting hydraulic fracturing activities on the pad. The site reconnaissance did not reveal any violations at the pad. The complaint investigation ultimately revealed that the damage observed at the complainant’s house was not related to activities at the Aldrich Pad, nor any other Oil and Gas related activity.
2352442 Primary Facility 2015-02-19 Routine/Complete Inspection No Violations Noted On 2/19/15, the Department conducted an inspection of the COG Aldrich L Pad in response to Cabot’s self-reporting of this small spill of recycled blended fluid. Present during this inspection was Jon Sassi, David Schoonover, Joe Gaboff, and Steve Watson of Cabot. Mr. Watson explained that Resource Environmental had determined that further remediation of the impacted area would need to be conducted. Mr. Watson stated that Cabot’s intention is to finish their scheduled hydraulic fracturing activities.
2377566 Primary Facility 2015-06-10 Administrative/File Review No Violations Noted The Department has reviewed and accepts the conclusions reached by Cabot in the attached Remediation Summary Report and associated Addendum relating to a small spill which occurred 2/15/15 on the Cabot Aldrich L. Pad. 2 field visits conducted by the Department on 4/16/15 by Jon Sassi and on 4/20/15 by Gene Rickard support the conclusions reached by Cabot. No further action needs to be taken by Cabot at this time.
2378722 Primary Facility 2015-05-28 Routine/Complete Inspection No Violations Noted Present were Gene Rickard, Jon Sassi, Rick Croll, Steve Watson of Cabot, and representatives of Resource Environmental, who Cabot has contracted to oversee the remediation of this apparent spill. During the inspection, the Department observed continued cleanup of the material in question. Mr. Croll scanned the remaining impacted area which had not been identified the previous day, and again found nothing of significance beyond background levels. The material did have a slight petroleum odor to it. Thus far 11 totes of material have been recovered. Mr. Watson estimates that if the material in question is contaminated, it would probably be around 3 barrels worth of material.
2378816 Primary Facility 2015-06-12 Administrative/File Review No Violations Noted On 6/12/2015 Resource Environmental on behalf of Cabot Oil and Gas submitted analytical results to the Department via e-mail regarding the discovery of a mud-like substance containing a slight petroleum odor on 5/27/2015. Resource indicated in the e-mail that a lubricating oil release had occurred at this location, a visual inspection of the southwest corner of the pad surface identified a brown soil-like material present as blotches on the surface and the material had a petroleum-like odor to it. Diesel Range Organics was measured at a concentration of 56,600 mg/kg. Total Petroleum Hydrocarbons was measured at a concentration of 89,200 mg/kg. The parameters are elevated above background levels indicating oil waste was released to and impacted the southwest corner of the pad surface.
2440721 Primary Facility 2016-01-13 Routine/Complete Inspection No Violations Noted On 1/12/16, Steve Watson, Cabot EH&S Specialist, contacted the Department to report the release of approximately 25 gallons of blended fluid on their Stellitano Pad. The release occurred at 7:40 PM that day when a hose clamp failed during a procedure to dewater a low point on the overland water line being utilized to transport the blended fluid from the COG Aldrich Pad to the fracturing operations being currently conducted on the Stellitano.

On 1/13/16, the Department met Mr. Watson at the spill site. Two technicians from Resource Environmental were on-site conducting characterization on the impact extent. Thus far it appears that most of the impact occurred on the access road itself. A gross amount of potentially impacted material has been removed and placed on containment. Resource is actively conducting soil conductivity testing as part of the remediation process.

2446305 Primary Facility 2016-01-07 Routine/Complete Inspection No Violations Noted Due to the transfer of recycled water from the Hibbard Facility to the Aldrich Pad for temporary storage and the transfer of fluid from the Aldrich Pad to the Stellitano Pad for use during hydraulic fracturing, it is apparent that a General Permit WMGR 123 Processing of Oil and Gas Liquid Waste is required and the Aldrich Pad is considered to be a residual waste transfer station. Recycled water is not being solely utilized for the wells on the Aldrich Pad and the frac tanks in the tank farm do not solely contain production fluid from the Aldrich wells. The conductivity was (3) milliSiemens/cm³ on containment in the freshwater section in the tank farm near the SE corner of the pad. Conductivity was (3.5) ms/cm³ in water in frac tank #251755; (5) ms/cm³ in frac tank #251757 and (8) ms/cm in frac tank #251807. These tanks are stage in the freshwater section of the tank farm. 2446875 Primary Facility 2016-01-08 Incident- Response to Accident or Event No Violations Noted Steve Watson, Environmental Health and Safety Specialist of Cabot Oil and Gas contacted the Department at 5:46 AM on 1/8/16 to report a spill of (15) gallons of recycled water. I contacted Mr. Watson at 8:30 AM on 1/8/16 to discuss the specifics of the spill. Mr. Watson stated that the spill occurred as a result of a Baker Corporation employee opening a valve on a frac tank not intended to be offloaded with recycled water transported to the site from Cabot's A&M Hibbard Centralized Treatment facility in Dimock, PA. Staining on the pad surface was observed in between a Diaz roll-of container lined with plastic and the corrugated pipe containment berm for the tank farm. The co-ordinates for the spill area are N- 41.78472°; W- 75.60059°. Resource Environmental was in the process of conducting conductivity soil electrical conductivity screening in the impacted area of the pad surface where the spill occurred. The recycled water is a mixture of flowback and production fluid generated from Cabot wells.
2448706 Primary Facility 2016-02-04 Routine/Complete Inspection No Violations Noted On 2/4/16, the Department received a report from Jim Garner, Susquehanna County Conservation District Manager stating that fracking operations being conducted by Cabot on their Aldrich and Stellitano Pads were causing mud to be deposited on State Route 92 at the intersection of SR 92 and the Great Bend Turnpike. During a site visit, a crew was observed monitoring this intersection and actively cleaning the intersection. No soil was observed on SR 92. This intersection is located approximately ½ mile from the access roads of both pads which are located on the Great Bend Turnpike. Mike Pirner, Cabot Construction Foreman was stationed at the Aldrich pad monitoring operations to include monitoring conditions of the unpaved Great Bend Turnpike. The Department finds that the fracking operations are not directly impacting SR 92. Mr. Garner was directed to contact either PENNDOT or the Township in resolving any future perceived issues at this particular intersection. No violations were noted during the inspec

2755631 Primary Facility 2018-07-18 Routine/Complete Inspection No Violations Noted The Department arrived on site at 1200 hrs to conduct a RTNC inspection. The Department was represented by Jon Sassi (EPS), Frank Hoehle (WQS) and myself.

Upon arrival, we observed that the E&S inspection log was up to date and that the required signage was present at the entrance to the pad's access road. The access road and pad surface were both solid and stable. The cut slope and fill slope were both well-vegetated and stable. All necessary PCSM BMPs were in place and in good condition.

The pad had nine producing wells at the time of our inspection: the 1H, 2H, 3H, 4H, 5H, 6H, 7H, 8H, and 10H. All the wells were properly labeled. No bubbling was present in the well cellar, but several of them were almost full to the top with stormwater and should be addressed promptly to prevent overflow, as seen in the pictures attached to this report. There were several cellars that were fairly full, but three of them are very close to overflowing and should be given priority: the 1H, 4H, and 10H.
