| Violations attached to 115-20715 -§691.402, §6018.301, §78.57.Incident Inspection for self-report of a release of (10) gallons of production fluid. Received the report on 2-2-13 at 1640 hours.  On 2-12-13  responded to the incident by conducting an investigation.  At the time of the investigation it was noted that an area was covered in plastic sheeting and several sandbags were noted on site.  No Cabot personnel were at the site during the investigation. Department spoke with Brody Webster on 2-15-13 to inquire about the circumstances of the release.  Mr. Webster explained that on 2-2-13 at 1500 hours that a release occurred. The 4” line off the GPU that runs to the production tanks froze.  It was determined by Cabot that the frozen liquid broke the line causing the (10) gallon release.eployed a water truck to vacuum the residuals and scrape up soil and frozen material which was staged on plastic.  GDS excavated an area approximately 10’X25’X2’ on 2-4-13,sampled on 2-4-13. Sample results elevated therefore they will excavate and resample.
