| On 1/27/15, the Department was on site at the Chief Taylor Well Pad to conduct a routine inspection.  One well has been drilled and is in production on this pad, the Taylor Unit 1H (115-20556).  Well site stability is good.  Pad is snow covered.  Production equipment functioning with no leaks or spills detected.  This inspection conducted in conjunction with an inspection of the Regency Taylor to Rogers Pipeline construction.  No violations noted.

| 1.  <70% perennial vegetation has been established.  As photographs 1 and 2 show, an annual rye is present that is essentially choking out the perennial vegetation.  Hence, permanent stabilization has not been achieved.  The Department suggests brush hogging the annual vegetation.

| On 1/4/17, the Department conducted an inspection of the Chief Taylor Well Pad located in Lenox Township, Susquehanna County to confirm Chief’s assessment that the site meets the minimum requirements as defined in Chapter 102.22(C) for permanent stabilization which would thus fulfill the physical requirements necessary for the permits termination.  Since the ESCGP permit application was received by the Department after November 19, 2010 on February 28, 2011, and since the well pad is to remain in full or partial existence, the Department inspected the following requirements related to this termination application:

2339375 Primary Facility 01/27/2015 Routine/Complete Inspection No Violations Noted On 1/27/15, the Department was on site at the Chief Taylor Well Pad to conduct a routine inspection. One well has been drilled and is in production on this pad, the Taylor Unit 1H (115-20556). Well site stability is good. Pad is snow covered. Production equipment functioning with no leaks or spills detected. This inspection conducted in conjunction with an inspection of the Regency Taylor to Rogers Pipeline construction. No violations noted.

2509927 Primary Facility 2016-08-11 Site Restoration No Violations Noted 1. <70% perennial vegetation has been established. As photographs 1 and 2 show, an annual rye is present that is essentially choking out the perennial vegetation. Hence, permanent stabilization has not been achieved. The Department suggests brush hogging the annual vegetation.

2. As evidenced in photograph 3, silt sock is still present on the west side of the pad. Hence, not all E&S BMP’s have been removed. The Department suggests removing the silt sock. 3. The as-built diagram does not reflect the approved permits restoration plan. Namely, the cut slopes for the access road going onto the pad are not represented on the as-built plan, and the fill slope that still exists on the northwest section of the pad housing the pipeline tie-in is not reflected in the plan. The Department suggests that if Chief wants to change the restoration plan to reflect the as-built, Chief should apply for a minor modification to their plan.

The Department’s determination is that the site is not permanently stabilized a

2552326 Primary Facility 2017-01-04 Site Restoration No Violations Noted On 1/4/17, the Department conducted an inspection of the Chief Taylor Well Pad located in Lenox Township, Susquehanna County to confirm Chief’s assessment that the site meets the minimum requirements as defined in Chapter 102.22(C) for permanent stabilization which would thus fulfill the physical requirements necessary for the permits termination. Since the ESCGP permit application was received by the Department after November 19, 2010 on February 28, 2011, and since the well pad is to remain in full or partial existence, the Department inspected the following requirements related to this termination application:

1. >70% perennial vegetation has been established. 2. All temporary E&S BMP’s have been removed. 3. The accompanying record drawing reflecting the as-built conditions submitted with the Notice of Termination application does not reflect the approved permits restoration plan.
