Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1981429 Primary Facility 06/10/2011 Routine/Complete Inspection No Violations Noted
1987234 Primary Facility 07/07/2011 Drilling/Alteration No Violations Noted Patterson 538'. In process of rigging down and moving to J. Hawley. Will be switching over to SOBM on next well. TD- proposed = 10531. Drillers log on site and up to date. Cemented 5 1/2" with 1780 sks of 15.5 ppg. Float held. Not cemented to surface. No violations observed.
2437903 Primary Facility 2016-01-05 Site Restoration No Violations Noted Inspection of the Smith R P3 Well Pad in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit.

The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. The restored footprint of the pad is less than, or equal to, the dimensions indicated in the plan. Post construction stormwater management devices were in place according to approved plans and appear to be functioning properly.

Based on the site inspection, I recommend this notice of termination be APPROVED.
