1811415 2009-07-07 No Violations Noted Drill rig on site is SWJ-19. Depth of drilling at time of inspection was 7332'. According to Company Man, the crew is drilling with mud using a 6-1/4 PDC bit. Crew will be drilling to a TD of 9534 and will set 4-1/2 casing at this depth. Drillers log was inspected and was filled out properly.

All E/S controls were in place and functioning properly. Freeboard in pit 1 was approximately 3', and pit 2 was approximately 10'. Liners in pits appeared to be in good condition at time of inspection.

1819275 2009-08-06 No Violations Noted Drill rig on site was SWJ-19. Depth of drilling at time of inspection was 9500'. At the time of inspection, the drilling contractor was cleaning out the hole. After trippin out of the hole, 4-1/2 production casing will be set. The Company Man was not on-site at the time of inspection.

There are currently two reserve pits located on site. One of the pits is located near the entrance to the well pad. At the time of inspection, the freeboard level in this pit was at the 2' level. The second pit, which is a tiered pit containing the shaker unit, and is located at the rear of the well pad. The freeboard in this pit at the time of inspection was aproximately 4'. Liners in both pits appeared to be in good condition at time of inspection. All E/S controls were in place and functioning properly.

1825895 2009-08-27 Violation(s) Noted My inspection revealed an un-permitted discharge of drilling mud occurred at the site while handling and transporting drilling hoses on location. This discharge is a violation of Chapter 78.54 of the Rules and Regulations of the Environmental Quality Board, 25 Pa. code, 78.54.

My inspection also revealed that drilling fluids were allowed to be released onto the ground which flowed across the well pad and over the well pad embankment. This un-permitted discharge of residual waste is a violation of Section 301 of the Solid Waste Management Act, 35 P.S. 6018.301 See inspection report for more details.

1841277 2009-10-30 No Violations Noted At the time of inspection, FracTech was on site preparing for the second stage of the frac job on the well. Upon arriving on location, the crew was addressing a diesel spill near one of the pump trucks on location. The diesel spill occurred from an overfilled saddle tank. Approximately 5 gallons of diesel appeared to have spilled onto the secondary containment ¿duck pond¿ located underneath the truck. The spill was contained within the containment area, and immediately cleaned up, and prevented from reaching the ground.

Prior to commencing the frac, FracTech completed a hydrostatic pressure test. During the initial pressure test, a small leak in one of the pipe connections was observed. The seal on the fitting was replaced, and the system was re-tested and passed. The second stage of the frac was completed over an approximate 3-hour period. During the frac no incidences occurred.
