|  At time of inspection, arrived on site at 12:50 p.m., the cellar was full of gravel, the (8x5) vent pipe was open and connected to the back side of the production line with 0psi on it. There was 65psi on the production casing and about 50psi on the production line. There was no gas detected at the perimeter of the cellar. I took a walk around the location and everything looked OK.

| Arrived at site 11:30. Weather: Clear, mild ~ 44 degs. The Pierson 1 well was the only well present on pad. Onsite in response to an application for Inactive Status for this well received on 11/3/17. It appears that this well was in production at some point. Sand separator, Prod. tank, well monitoring "SCADA" devices, production metering and sending units were all present on pad. Appears that gas production unit "GPU" has been removed. - Insp. of wellhead showed: This is an older vertical well drilled on 11/24/2008. Well cellar area was filled with gravel. Gas checks were performed using a ALTAIR 5X gas meter around wellhead flanges, fittings and several areas of gravel around wellhead using a ground probe. No gas was detected at areas checked. - I will check previous prod. reporting, mechanical integrity reporting and validity of existing well permit upon return to office. Left site at 12:30. All Production and MIA reporting was up to date. I will be recommending approval of inactive status for this well.

| The Department arrived on site at 1330 hrs to conduct a RTNC inspection. The Department was represented by Frank Hoehle (WQS-DEP) and myself.

Appropriate signage was displayed at the entrance to the pad's access road. The access road itself was stable and solid, with no apparent erosional issues.
 +

The pad was not located on a hill, so there was no cut slope or fill slope. However, there were berms present around the perimeter of the pad that were stable, solid, and well-vegetated. The pad's surface was also solid, even containing vegetation in some areas.
 +

The pad only contained one vertical well which was not currently producing any gas. The landowner, Mr. Pierson, showed up while we were conducting our inspection and informed us the well had had not been in production for some time. Mr. Pierson also said that as far as he was aware, Chesapeake did not have any intention of resuming production on this site.

2463836 Primary Facility 2016-03-11 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:50 p.m., the cellar was full of gravel, the (8x5) vent pipe was open and connected to the back side of the production line with 0psi on it. There was 65psi on the production casing and about 50psi on the production line. There was no gas detected at the perimeter of the cellar. I took a walk around the location and everything looked OK.

I left the site at 13:30 p.m.

2661704 Primary Facility 2017-11-15 Routine/Complete Inspection No Violations Noted Arrived at site 11:30. Weather: Clear, mild ~ 44 degs. The Pierson 1 well was the only well present on pad. Onsite in response to an application for Inactive Status for this well received on 11/3/17. It appears that this well was in production at some point. Sand separator, Prod. tank, well monitoring "SCADA" devices, production metering and sending units were all present on pad. Appears that gas production unit "GPU" has been removed. - Insp. of wellhead showed: This is an older vertical well drilled on 11/24/2008. Well cellar area was filled with gravel. Gas checks were performed using a ALTAIR 5X gas meter around wellhead flanges, fittings and several areas of gravel around wellhead using a ground probe. No gas was detected at areas checked. - I will check previous prod. reporting, mechanical integrity reporting and validity of existing well permit upon return to office. Left site at 12:30. All Production and MIA reporting was up to date. I will be recommending approval of inactive status for this well.
2736883 Primary Facility 2018-05-25 Routine/Complete Inspection No Violations Noted The Department arrived on site at 1330 hrs to conduct a RTNC inspection. The Department was represented by Frank Hoehle (WQS-DEP) and myself.

Appropriate signage was displayed at the entrance to the pad's access road. The access road itself was stable and solid, with no apparent erosional issues.

The pad was not located on a hill, so there was no cut slope or fill slope. However, there were berms present around the perimeter of the pad that were stable, solid, and well-vegetated. The pad's surface was also solid, even containing vegetation in some areas.

The pad only contained one vertical well which was not currently producing any gas. The landowner, Mr. Pierson, showed up while we were conducting our inspection and informed us the well had had not been in production for some time. Mr. Pierson also said that as far as he was aware, Chesapeake did not have any intention of resuming production on this site.

The single well present on the pad was clearly and properly labeled with its name and permit number
