1833423 2009-09-25 No Violations Noted An existing road was used to access the well site. The Site ID sign and E/S Plan were present at the entrance of the access road. At the time of inspection, the drill rig was offsite. The area around the well has been reclaimed. The well is producing.

Standing water was present around the base of the well. Based on visual bubbling, gas seepage was observed between the 7" and 9-5/8" casings of thewell.

1835385 2009-10-14 No Violations Noted The inspection was completed as a follow up of a previous inspection performed on 9/25/09 where gas seepage was observed in between the 7" and 9-5/8" casings of the well. During this inspection, a small amount of standing water was present around the base of the well. Minor gas seepage was observed between the 7" and 9-5/8" casings of the well.

2139922 2013-02-14 No Violations Noted On site at 10 am to collect pressure reading and isotopic gas sample from annulus of well. DEP reps on site were Steve Watson, Eric Rooney, and Gene Rickard. DEP met with Mr. John Smelko of Cabot. Selman reps on site to assist in gas readings and to split gas samples. Cabot shut in annulus at 9 am this morning. At 10:23 am a digital pressure gauge was installed on the 7 x 4" annulus. Pressure = 23.7 psi. Gauge was removed and open vent was screened for gas. Model 60 reading = >5% combustible gas, and recorded 100% LEL. Prior to sampling, fluid was drained from vent line. Both DEP and Cabot collected gas samples for isotopic analysis. After completion of sampling, Cabot re-connected vent line and locked vent open. Cabot is planning on moving in a workover rig this week to service well.
