Spoke to PIC Larry Dobbs who informed DepT that drilling for 5H had begun morning of 11/27/12.At the time of this inspection, actively drilling 5H, 1H had been drilled, cellars for 2H,3H,4H constructed, 6H staked.Minor erosion on hillside observed on back left corner of pad when facing wellheads from access road. Minor sediment from this erosion observed at base of hill in rock-lined ditch.Observed tophole cuttings along soil pile. Dept spoke with company rep who stated plans were to incorporate cuttings into soil pile and disperse across pad.Opr will address erosion concerns by routinely monitoring this area in order to prevent pollution event due to accelerated erosion and sedimentation.Access road adequately rocked and no E&S issues were noted in relation to access road.Department recommends that Operator continues to monitor and maintain access road and well-site as necessary.

2130320 2013-01-14 No Violations Noted At the time of inspection, no E&S issues were present at the site. Chevron was setting the water protection pipe (16”) to a depth of 415 feet. Coal was encountered at 575 feet to 586 feet. Coal protection pipe (11 ¾”) was set at 581 feet. Cement basket was set at 556 feet. Pipe was cemented with 328 sacks of Class A and had a return cement of 8 bbl. The current depth was 1,837 feet. Chevron site supervisor was David Freed.
2136134 2013-02-05 In Compliance with Policy Admin Inspt. Spoke to Chevron representative Curtis Small via phone on 2/5/13. According to Mr. Small, on 1/23/13 a jug of antifreeze was spilled in bed of truck as it was being driven down access road. It leaked through the tailgate, resulting in a one-inch, 70 foot strip of antifreeze down the AR, totaling approximately a half-gallon in volume. Impacted gravel immediately shoveled into drum. Samples taken, will dispose of in landfill. It appears Operator handled situation according to policy and a follow-up inspection will be conducted in future to ensure full compliance.
