2121597	2012-11-28	No Violations Noted	Inspection result of spud notification. Multi-well pad. Site ID sign, valid permit, E&S plan, ESCGPI, PPC Plan present at access road entrance. Security on-site. At the time of this inspection, cellar for 2H constructed. Actively drilling 5H, 1H had been drilled, cellars for 3H,4H constructed, 6H staked. Minor erosion on hillside observed on back left corner of pad when facing wellheads from access road. Minor sediment from this erosion observed at base of hill in rock-lined ditch. Observed tophole cuttings along soil pile. Department spoke with company representative who stated plans were to incorporate cuttings into soil pile and disperse across pad. Operator will address erosion concerns by routinely monitoring this area in order to prevent pollution event due to accelerated erosion and sedimentation. Access road adequately rocked and no E&S issues were noted in relation to access road. Department recommends that Operator continues to monitor and maintain access road and well-site as necessary.

2127509	2012-12-17	In Compliance with Policy	Admin Ispt. Spoke to Chevron representative Curtis Small via phone on 12/17/12. According to Mr. Small, one quart of frac lubricant was spilled on 12/16/12 and it was immediately removed from the ground and placed with the cuttings. It appears that Operator handled the situation according to policy and a follow-up inspection will be conducted in the future to ensure full compliance.

Water Protection pipe (16”) was set at 413 feet and was cemented with 421 sacks of Class A with 17 bbl. of return cement. Coal protection pipe (11 ¾”) was set at 648 feet. Cement basket was set at 562 feet. A total of 340 sacks of Class A cement were used on the pipe with 19 bbl. return.
