| Inspected the site on 8/20/13. There was not any activity on the restored well pad. The road going into the site was well graveled. The welll pad was well graveled. The Well pad signs were properly marked. This pad has three producing Marcelus gas wells. The three wells were properly marked.The drums observed  in the past inspection were removed from the pad.There were two dumpsters on the well pad that contained Demo waste. No staining on the well pad was observed. The fresh water impoundment is now fenced in. No E&S issues were observed. During the inspection no issues were observed that need to be addresed

2069383 2012-05-10 No Violations Noted At the time of this inspection, site Id sign, Es plan, ESCGP-1, and PPC plan onsite. Site gated and monitored by 24hr security. Multi well pad. Crown Rig # 8 actively drilling 6H. Charles, DSM onsite during this inspection. Secondary containment in place beneath rig and onsite tanks. No pits installed by Chevron for drilling operations. Tanks and secondary containment look structurally sound. Appears bmp's are installed and functioning according to plan. No accelerated erosion or sedimentation issues noted with site or access road. Access road adequately rocked. Noted a "small" leak coming from Residual waste box on site, notified DSM whom stated the problem would be addressed immediately. Absorbent pads were placed in location of leak. Department also noted to DSM of a possible safety concern near back side of rig, where it appears shale was placed over top electric lines and drilling equipment (see picture). DSM stated that matter would be addressed immediately . nO VIOLATIONS NOTED.

2092741 2012-05-15 No Violations Noted At the time of inspection, Chevron was drilling the horizontal leg of gas well 6H. Crown #8 rig present. Total depth was 11,014 feet with kick off at 6549 feet. Halliburton performed cementing of the production pipe. Cement used was lead 310 sks varicum (lead), 1,395 sks gas stop (tail) and completed with 420 sks Frac Chem. 40 bbl. of cement was return to surface. During the site visit, Chevron had a release of 1 bbl. Of oil base mud on the plastic containment near the disposal boxes. Crew removed the liquid portion and was adding absorbent material to the plastic. Talked to Chad Eiserman (Chevron) about the release of oil based mud.

Brine tanks on steel containment, both appear structually stable. Site temporarily stabilized. Access road stable, adequately rocked. No accelerated E&S issues observed in relation to well pad and access road. According to records, as of 8/21/2012, 3H, 4H, and 5H were all proposed, but never materialized. Department recommends that Operator update site ID sign to ensure that only wells with active permits are listed.
