Conductor pipe set 20¿ pipe at 78 ft. from grade (ffg). 13 3/8¿ pipe at 1,014 ft from grade used 725 class A sacks and 9 5/8¿ set at 2,004 ft used 700 sacks of class A sacks. Running second geophysical logging event after hole was deepen from 8960 ft to 9123 ft. According to Chief, roughly 500 ft above Marcellus, Chief will fill hole up to that point with cement and set a kicker. Pit aligned with 20 mil plastic and no free board issues.

Previous attempts to restore were evident, but site was actively being reseeded to ensure uniform vegetative cover in areas that did not achieve 70% growth. Pits have been removed and area restored. Rock pad. Access road was in stable condition. No accelerated erosion or sedimentation issues were observed in relation to access road or site. It is recommended that operator continue to maintain all bmps until final (uniform) restoration measures are achieved. Sub- contracting rep (Keystone) Josh was onsite during inspection. A follow up inspection will be conduction to ensure reseeded areas sustain 70% vegetative growth and site is permanently restored/stabilized.
2004335 2011-09-13 No Violations Noted During this inspection, Chevron Regulatory Compliance Personnel and WQS Marc Maestra present. Well ID sign, ES plan w/ PPC plan, and ESCGP-1 onsite. Site gated and locked. Access road entrance and roadway stable. Well drilled not yet in production; recently granted inactive status. Well head lined with felt and plastic-noted red staining on liner below well head-operator plans to ensure structural integrity of well head is not compromised¿may be result of rust. No pits located onsite, although one frac tank and two brine tanks on pad- both secured by secondary containment. Appears rainwater has begun to accumulate in containment unit due to recent heavy rains; department is recommending operator properly dispose of water from unit. Department recommends operator remove secondary containment liner (placed on pad) previously used during frac operation.

Pipeline from Bowers West impoundment utilized for frac operator currently disconnected due to pipeline break that occurred on 2/20/11, (see incident follow up inspection # 1952923). No visible evidence that previous line break adversely impacted this site, at the present time. No violations noted onsite. Site had previously been reseeded and mulched, but operator should continue to monitor site/access to prevent the potential for accelerated erosion or sedimentation concerns due to recent thaws and increased traffic.
1965630 2011-04-14 No Violations Noted At the time of inspection, site id sign, ES plan. Site gated with 24hr security guard. Access road stable. Well recently fraced, containment pad (felt /plastic lining placed below well location). No pits onsite, three frac tanks that appear to be sound, as well as 8 covered containment boxes. Temporary bmp¿s in place and functioning noted that previously disturbed areas have been reseeded and mulched with approximately 70% vegetative cover. No accelerated erosion or sedimentation issues observed on site or access road, although operator should continue to closely monitor all bmp¿s until final permanent site restoration is achieved. No IW issues noted.
