| Well site is in active drilling status.  Freshwater pit is embedded properly and is maintaining freeboard requirements.  There are no signs of erosion and sedimentation on the well site or access road.  The operator should continue to monitor the access road and well site for erosion and sedimentation until final restoration activities are complete.  The BMP's that are in place should be maintained.  Filter socks are a temporary BMP that should be monitored to ensure they are intact and functioning. This is a MARCELLUS WELL.

Freshwater pit is embedded properly and is maintaining freeboard requirements. There are no signs of erosion and sedimentation on the well site or access road. The operator should continue to monitor the access road and well site for erosion and sedimentation until final restoration activities are complete. The BMP's that are in place should be maintained. Filter socks are a temporary BMP that should be monitored to ensure they are intact and functioning. This is a MARCELLUS WELL.
1800885 Primary Facility 2009-05-28 Drilling/Alteration No Violations Noted During inspection, DEP met Samson representative, Charlie Howard. Frac project started on 5/27/09 and continued throughout night shift. Water flow to pit occurring at the time of inspection. No flaring occurred. Well head pressure at 523 psi and Samson was performing water sampling of the discharge at the time of inspection. At the time of inspection, no E&S issues and no IW discharge issues.
1812026 Primary Facility 2009-07-10 Drilling/Alteration No Violations Noted During inspection, DEP met Samson representative, Charlie Howard. Frac project started on 5/27/09 and has continued to date. Well was being sparged to bring out the brine water into tanks. Emission gas was being flared. Brine and Frac water was currently recycled via on-site treatment system (Ozone Technology). The frac water was pumped to above ground treatment tank where the water was agitated and treated with O3 and 10 % hydrogen peroxide. Upon completion of treatment, water is transferred to on-site frac tanks and shipped to the Troutman #1 site for future frac project. At the time of inspection, no E&S issues and no IW discharge issues and no freeboard issues. Pit is aligned with plastic
1821126 Primary Facility 2009-08-13 Routine/Complete Inspection No Violations Noted The pits on site have been restored. Drilling is complete, but well is not in production. There are no signs of erosion and sedimentation on the access road and well site at the time of the inspection.
1837093 Primary Facility 2009-10-08 Routine/Complete Inspection No Violations Noted At the time of inspection, the well was drilled. Drilling concluded on 03/1/2009. The access road was in good condition. Compost socks were in place; no accelerated E&S issues were noted. Pits had been removed. The site had not yet been restored, as the area had not be seeded or mulched. Note: I received notice/letter from inspector Paul Wyrwas that Samson is attempting to file for an extension on restoration timeframe in order to reevaluate the well; therefore, Samson has put restoration proceedings on hold until further notice.

Dept received restoration report February 2010. Well site had not been completely restored as of this inspection. Drilling was completed on March 1, 2009; therefore, site was not restored within nine months of completion. Pad was rocked, pits were removed, but area not been seeded or mulched; therefore, 70% successful vegetative growth was not observed. Bmp filter sox¿s were surrounding site and functioning as a temporary bmp no accelerated E&S issues were noted. Observed soil pile had not yet been removed, but was stabilized.

Pad in place; no production equipment installed yet; perimeter BMP¿s removed and site revegetated with minimum 70% vegetative cover. Access road maintained and no E&S problems noted.

Well plug in accordance with 78.92 regulations. 11 ¾ ¿, 42#, H-40 csg. @ 500¿, 8 5/8¿, 32#, J-55 csg. @ 1,512¿ (245 sks Hal Lt &185 sks Class A),5 ½¿ , 20#, P-110 csg. @ 6,001¿TOC 3,838¿ (70sks of Extendacem scav & 235 sxs Varicem) PBTD ¿ 5,808¿. Completed interval for Marcellus Shale: 5,523¿-5,527 and 5,594-5,602 (60 holes) and 5,681¿-5,685¿ and 5,699¿-5,709¿ and 5,714¿-5,716¿ (79 holes ) Remove of BOP stack and set mechanical plug at 5500 ft. Process of setting cement plug of 100 ft on top of plug. (17 sks of class A). 0 psi on well head prior to working on well. No pit was completed. Samson using containers for water from well and potable water. At the time of inspection, no E&S issues and no IW discharge issues
1965621 Primary Facility 2011-04-14 Routine/Complete Inspection No Violations Noted At the time of inspection, site ID sign and Es plan present. Drilling completed 03/01/09. Well plugged and marked with erected monument with API number attached. Pad not yet restored, pits removed (1 frac tank onsite) and previously disturbed areas seeded mulched with >70% vegetative growth. No accelerated erosion or sedimentation issues noted with site or access road. Spoke to land owner onsite, who inquired about deadline for restoration--- reiterated regulation requires that within 9 months from date of completion of plugging the site is to be permanently stabilized by >70% perennial vegetative cover or other permanent stabilization measures capable of resisting erosion--- specifics are between land owner (ease agreement) and operator as owner would like to farm field. Also stated that erected monument will stay in placed as required by regulations. No violations noted at this inspection.
