During inspection, the well tag was present. As indicated by the pictures, industrial waste has discharged from the brine holding tank to the ground. Oil sheen located in the well head sump and around the smaller holding tank. The smaller tank was 1/4 filled with brine fluid. Water from well head sump discharged to field. Brine tank (larger tank) showed multiple holes on top and sides of tank . Tank also showed signs of corrision on the bottom of tank. Tank was 3/4 filled with fluid. Staining on the side of larger holding tank. Brine tank was heavily corrided at the bottom of tank. The well was completed with 13" flange well assembly retrofitted 562162

1808522 2009-06-22 No Violations Noted During inspection, the well tag was present. Industrial waste has discharged from the brine holding tank to the ground has been removed. Oil sheen located in the well head sump has been removed. New 100 bbl poly brine tank has been installed. Old brine tank has been removed from the site. No E&S or IW discharge issues at the site. Access road is heavily eroded for steepness of grade and requires additional maintenance.
1814079 2009-07-20 Violation(s) Noted Drilling was completed on 11/15/2003. Site has been restored. Brine tank is in sound condition. The access road is on a very steep slope with no BMP's to control erosion and sedimentation. As a result sedimentation has occurred in a tributary of Beaverdam Run. The department reccommends that BMP's be utilized to minimize accelerated E&S on the access road. An example of a BMP that may accomplish this goal would be: Install 2 water bars with rip rap outlets on the straight steep section of the road and a sediment trap at the bottom of the slope before discharging into the waters of the commonwealth.See attatched notice of violation and respond as requested. 1893233 2010-06-10 No Violations Noted At the time of inspection, well was drilled, tagged, and in production. Brine tank appeared sound. Site was restored. Access road was stable. No accelerated erosion and sedimentation issues were noted in relation access road or well site. It is recommended that operator continue to closely monitor access road E&S controls due to the roadways steepness.
