| On July 25, 2017 I conducted a routine inspection of the West Shanksville Saltwater Disposal Well.  Matt Gary, Field Supervisor, was present from Cottonwood Operating Company a subsidiary of LPR Energy, LLC.  Michael Morgart was also present from DEP.  According to Mr. Gary fluids had been injected on the morning of the inspection but had stopped by the time we arrived.  The old brine storage tanks have been cleaned and removed.  They were staged onsite awaiting potential recycling options for the scrap metal.  The pump building was observed.  Scale inhibitor is added prior to injection.  Used filter socks were observed stored in leaking cardboard boxes.  Cottonwood is recommended to properly store and dispose of solid wastes generated during the operation of the injection well.  A series of Frac tanks are being used to temporarily store produced fluids until the tanks purchased from Chevron can be installed.  A new secondary containment structure was being assembled.  Biocide is added as necessary to the fr

Inspected telemetry system and failsafes.  Inspected site.  No issues at the site during inspection. Departed site at 10:50 hours.

2622156 Primary Facility 2017-07-25 Routine/Complete Inspection No Violations Noted On July 25, 2017 I conducted a routine inspection of the West Shanksville Saltwater Disposal Well. Matt Gary, Field Supervisor, was present from Cottonwood Operating Company a subsidiary of LPR Energy, LLC. Michael Morgart was also present from DEP. According to Mr. Gary fluids had been injected on the morning of the inspection but had stopped by the time we arrived. The old brine storage tanks have been cleaned and removed. They were staged onsite awaiting potential recycling options for the scrap metal. The pump building was observed. Scale inhibitor is added prior to injection. Used filter socks were observed stored in leaking cardboard boxes. Cottonwood is recommended to properly store and dispose of solid wastes generated during the operation of the injection well. A series of Frac tanks are being used to temporarily store produced fluids until the tanks purchased from Chevron can be installed. A new secondary containment structure was being assembled. Welder arrived and rigged up, started cutting away at flange bolts Torched off both top and bottom bolts, tried to lift flange with rig at 40k w/ no luck Welder still cutting away at bolts and making headway, pulled again with rig and popped B section off Welded up 5' pup to 5 1/2 casing to pull tension and remove slips from A section Started pulling on 5 1/2 w/ rig at 90k tension, no movement, beat on wellhead, tried again w/ 90k tension, still no movement from slips, unloaded 2 3/8 drill pipe Had welder cut into wellhead, beat on well again and finally broke slips free Cleaned up wellhead and cellar, prepped for welding on new wellhead tomorrow morning, shut down for the night.

Called in wireline and determine the pipe size diameter was smaller. Wireline also determined No obstruction to 4600 feet where the packer is located. LPR ordered a smaller diameter mill tool. Currently waiting on tool. Should start to mill this Thursday evening and go into the night and Friday. Afterwards, LPR will Run the wireline for rest of well.

RJ well services (RJ) has completed the following: replaced well head, milled the packer out and flushed well and completed pump house building construction . Currently, RJ is setting up to run the wireline to test for bond log, magnetometer, and gauging caliper. After results, LPR will determine the location of packer and perform acid wash of Orkisky formation and complete well and site preparation for injection. RJ scheduled for approximately two weeks to complete all activities.

No issues at the site during inspection. Departed site at 10:50 hours.
