2630663 Primary Facility 2017-08-29 Site Restoration No Violations Noted On 8/29/17 at 1358 hrs I conducted a restoration inspection of the Foothills Sportsman Club #1 well. No one was present at the site. It was partly sunny and 67 degrees F.

The #1 well was drilled by Allegany Enterprises in April 2010. The well was tagged and the area around the wellhead mowed. Production equipment was observed in earthen containment adjacent to the well. The site is restored and permanently stabilized as per 25 Pa Code Section 102.22(a). The Well Site Restoration Report (WSRR) was received on 5/4/17; it was reviewed and acknowledged on 8/29/17. According to the WSRR, an encapsulated drill cuttings pit is located 25 feet north of the wellhead.

Ownership of the well was transferred from Allegany Enterprises to Roulette Oil and Gas on 5/31/17.

No violations were cited during the inspection. I departed the location at 1408 hrs. TLT

All wellhead assembly fittings and flow lines were tested with a gas meter with the results of this testing being no gas detected on wellhead assembly or well area.
2719981 Primary Facility 2018-04-17 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. The well head assembly,fittings,vents and flow lines were tested for combustable gas. Results of this testing was that no gas was detected.

2853304 Primary Facility 2019-03-13 Routine/Complete Inspection No Violations Noted At the time of this inspection this well appeared to be a producing well. Well is approximately a 7" x 4" x 2" x 2" solid body tee (t) style fitting that s he wellhead assembly. The approximate 7" diameter is the bottom of the fitting that then reduces to approximately 4" with an approximate 2" tee opening on both sides of the fitting. One side of the tee style fitting has a valve (shut) with a plug in the valve. The other side of the tee style fitting has a 2" valve (open) that connects to a production / flow line that goes into the ground. The flow line then resurfaces and gos into production equipment. A filter/ dryer (desiccant) tank then a separator and then a meter and back into the ground. A gauge on the well head assembly was showing a line pressure of 55 psi( pounds per square inch). No violations noted at this inspection.
