2695930 Primary Facility 2018-02-14 Routine/Complete Inspection No Violations Noted There are six wells permitted in Project KML-2. Of those six, three permits were cancelled shortly after being permitted in 1978. The three remaining wells: 1J (085-20151), 3J (085-20153), and 6J (085-20156) remained categorized as 'Active' and have been transferred to the current operator, Millennium Oil (G. Tharan). Other than the Well Location Plat, no Well Records or other documentation exist on file for these wells.

Well locations were projected from the Well Location Map, Project KML-2, dated 2/16/1978. WQS did inspections at the projected locations to verify if the wells were, or were not drilled.

At projected location for 1J (085-20151), WQS found 8-inch conductor covered by a rock. A large rock is jammed in the conductor 5' down. On 2/16/18, WQS inspected casing a second time with the operator and Oil and Gas Inspector Tom Gould, at which time it was determined that because of a lack of well record documentation and equipment on site, it is likely that this is 8 inch conductor (drive pi
