2043989 2012-02-16 No Violations Noted Kriebel had contracted Falcon. E&S controls and well sign were present. No freeboard issues and no E&S issues were present at the time of inspection. The Surface/Water protection casing (7”) was set at 1,612 feet and was cemented with 285 sacks of Type I. No cement return was recorded on the 7” piping. According to Steve Pavlchick, Kriebel was going to cement the production string to surface. Plug was down on 00:00 hrs.’ on 2/14/12. Falcon was currently drilling at 2, 850 feet.

Well was drilled and frac'ed. Kriebel rig was running tubing during inspection. Access road and pad was muddy but stable. Both pit liner and impoundment liner were held in place by either metal or wooden stakes through the liner. This construction method comprimises the integrity of the liner. Pit liner should be back filled and not staked. Sed fence was used as the BMP. It also had been comprimised. Pressure from fill against the fence caused fence to tear in numerous places. Also the sed-fence was not properly installed (not properly trenched) in some spots. No accelerated erosion was taking place. These are recurring issues with Kriebel well pads and have been noted in previous inspections.
