Recommend operator place ESCGP-1 in box with ES plan, as unable to locate at time of inspection. Access road stable and adequately rocked. Pioneer drilling Rig 60 onsite rigging up, not yet drilling. Top hole drilled. Pad beneath rig lined with secondary containment, as well as all areas containing onsite liquids. One lined pit located onsite containing rainwater, adequate freeboard although dept recommends emptying prior to drilling. Bmp﾿s installed and functioning according to plan; no accelerated erosion or sedimentation issues noted. No violations noted during this inspections.
2017855 2011-11-09 No Violations Noted Site visit was in response to incident notification by Nick Staffieri (Chevron) to Ed Ritzer (PA DEP Water Qualiy Specialist Supervisor at 07:43 on 11-9-11.Spill occurred do to the following: Haliburton had been cementing the well. Well was being cemented at 8 gpm. This rate caused an overflow of oil based mud at the well head. This overflow was not immediately recognized. Multiple levels of containment were breeched and the mud made it to the surface of the pad. A small area of stone on top of felt lined material was contaminated with this material. Most of the mud had been removed at the time of inspection either by vac'ing or through absorbents. Crews continued to lay absorbent material on any spilled material that had not yet been removed. Absorbent material was then being removed for proper disposal.
2020926 2011-11-18 No Violations Noted Site visit was follow-up to 11-9-11 incident inspection to mud based oil spill.

Rig had been moved to second hole on pad. Secondary containment had been fixed and reinstalled. Site was frozen and muddy, but did not appear to have any residual mud left on the pad. Site will be revisited when rig is removed to insure no residual oil is left on the pad. BMPs were in place and working properly. No E&S issues. Site inspection was conducted with Jesse Olson (PA DEP Inspector). Pat Patterson was company man on-site.

2035949 2012-01-11 No Violations Noted During this inspection, Site Id sign located at access road entrance. ES plan, PPC and ESCGP-1 located onsite. Pioneer Rig 60 onsite actively drilling, 8H previously drilled and top hole drilled on 10H. Pad lined with secondary containment, which appears structurally sound. All areas containing liquids and or hazardous materials secured on containment. Noted that pit levels are approaching 2ft of required lined freeboard. Department spoke to Company man(Brandon) onsite and recommended that pit be pumped in order to prevent a possible pollution event. Also spoke with Chevron Regulatory Compliance personnel via telephone regarding pit freeboard concern. Temporary bmp's installed and functioning properly. Operator shall continue to closely monitor and maintain pit freeboard levels as well as bmp's. No accelerated erosion or sedimentation issue noted. Access road stable. Site gated and protected by security guard. No violations noted during this inspection.

2037025 2012-01-24 No Violations Noted On Sun 1-22 at approximately 1530 a reported release of ~ 2 barrels of synthetic based drilling mud on to secondary containment located beneath rig. Today, spoke with Company man Brandon whom stated the release (he believes) was actually < 1 barrel occurred while pumping mud from frac tanks into shaker. Due to temperature change mud became viscous and clogged pump which back fed mud and ultimately lead to release onto containment. All released mud is believed to have been contained on secondary containment &immediately removed. No evidence of adverse environmental impact noted at time of this inspection. Based on above statement and visual inspection it appears that all mud was contained and immediately disposed of. Although some areas of the containment are missing berms and/or berms are in need of maintenance the area of release appears structurally sound and appears mud contained on containment. Follow up will be conducted upon removal of rig &containmnet to confirm mud release did not adversely impact grounds of commonwealth below contain

2045561 2012-02-23 In Compliance with Policy Inspection in response to notification of incident involving the release of approximately 3 gals diesel from pump truck. Well site Id sign, ES, ESCGP-1, and PPC plan present. Well tags present. Well(s) drilled not yet in production. One pit located onsite with sufficient freeboard. Spoke with Chevron compliance rep Curtis Small, whom stated that it is believed a check valve on the pump truck failed and while sitting on a grade the diesel line released approximately 3-5 gals of diesel on pad until truck driver realized a line was leaking. Driver immediately addressed the faulty valve/line, and operator began immediate cleanup. A dike was dug to contain release and then a visual cleanup of diesel stained areas began. During todayï¾’s inspection, operator had removed contaminated soils and placed in roll off box located onsite, it was also noted that environmental services lab currently onsite collecting soil samples to ensure all contaminated soils have been removed. Awating sample results from operator.

2069243 2012-05-03 No Violations Noted nspection result of follow up to incident that occurred on 4/18/12 on Buterbaugh 8H (Inspection ID #2061749), reported release of 3-5 bbl of freshwater/friction reducer (residual only) combination onto and outside of containment. Security onsite. Well gated. One below grade drill pit located onsite with adequate freeboard. Secondary containment placed below well heads, two onsite tanks, and roll off box. Recommend operator remove piece secondary containment located on pad near onsite tanks. Minor ponding of water located on site due to recent heavy rains. Bmp's installed and appear to be functioning no accelerated erosion or sedimentation issues noted. Previously disturbed areas have been seeded mulched with observed growth. Site not yet permanently restored.

Site monitored by security guards. Multi Well pad, Three wells drilled not yet in production. All labeling present on well heads. Access road entrance paved. Well site stabilized with adequate rock and vegetative mats installed in drainage ditches. Previously disturbed areas adequately vegetated and temporarily stabilized. Rig onsite drilling out plugs. Operator plans to flow back into tanks and/or pit. Currently pit is structurally sound, but is approaching freeboard. Operator agreed to pump out pit contents prior to beginning flow back operations. Secondary containment placed underneath rig appears. No accelerated erosion or sedimentation issues noted at the time of this inspection. Reminder that operator has 9 months from completion of drilling of last well (9H), on multi well pad to restore unless other approval is deemed from the Department as per current regulations. No violations noted at the time of this inspection.

2109188 2012-10-15 No Violations Noted Inspection result of request for restoration extension. Department received a request for restoration extension on October 4, 2012 (see attached) via email. As per regulation the current (9 month) restoration Deadline is October 26, 2012, based on completion of drilling of last well, on multi well pad, of 1/26/12. At the time of today's inspection, site ID sign, ES plan, ESCGP-1, permit, and PPC plan located onsite. Site monitored by 24-hr security. Wells drilled not yet in production. Site has been temporally stabilized with bmp's and/or rock. No accelerated erosion or sedimentation concerns noted. One below grade lined drill pit onsite. During this inspection Waste Management (independent contractor) and Chevron personnel were actively removing contents (liquid and cuttings) from pit via vac truck. Liner appears structurally sound and freeboard level maintained. No industrial waste concerns were noted at the time of this inspection. It appears that water and liner have been removed from freshwater impoundment. No viols. Restr ext granted
2137767 2013-02-05 In Compliance with Policy ADMIN INSPT- At approximately 1355 operator call SWRO to report spill and then call WQS.As per operator at approximately 1300hrs a service truck was repairing a flat tire on truck on well site backfilling drill pit, when a hydraulic line broke on repair truck and discharged ~1 gallon of hydraulic fluid onto snow covered/frozen pad. Chevron rep stated that a containment dike has been built surrounding release. The area has been covered with secondary contain and foam triangles, the contaminated "snow" will be placed within dike and covered. The area is highly marked to prevent third party disturbance and or coverage due to additional snow. On 2/6/13, contractors plan to remove impacted soils, place in sealed drums until final dispostion. Per opr spill quickly contained, minimal environ impact. Written response will be provided on what happend and clean up measures undertaken. Follow up inspection will be conducted to determine extent of environ.impact and examine clean up measures as well as restoration of impacted areas
2137909 2013-02-06 In Compliance with Policy Incident occurred near impoundment where crews were utilizing soil pile to fill in drill pit. Appears when hydraulic line broke from truck it sprayed ~ a gallon of fluid over a broad area of frozen/compacted snow It does not appear that the fluid puddled up or ran off site, nor does it appear to have infiltrated into the frozen snow, although a follow up inspection (upon completion of clean up) will be conducted to ensure no adverse environmental impacts were sustained as a result of this minor release. As per onsite contracting crew (Penn Mechanical) plans were to removed contaminated snow pile (placed on and covered with secondary containment liner) and place into barrels prior to final disposition. According to reps onsite plans were to remove snow later this afternoon. Department cautioned operator that higher temperatures are in the weather forecast; therefore they should expedite the cleanup process in order to prevent the potential for a subsequent pollution event.
2141385 2013-02-12 No Violations Noted ollow up inspection to reported release of hydraulic oil on 2/5/13. Department currently waiting on "official" spill report from operator. At the time of this inspection, all impacted/contaminated snow has been successfully picked up and contained in barrels placed on secondary containment. No adverse environmental impact ( i.e. to soils, grounds of the commonwealth) noted at the time of todayï¾’s inspection. Site has been adequately (temporarily) stabilized, no accelerated erosion or sedimentation issues noted. No violations noted operator appears to have appropriately and adequately responded to release according to current department policy. Department will follow up with operator to ensure that materials are properly disposed of.

No accelerated erosion or sedimentation issues observed in relation to well site or access road.Pad size minimized due to production equipment placement and on-site conditions.Upon adequate vegetative growth, sed trap and ditches will be filled in.Department recommends Operator continue to monitor and maintain well site to ensure final restoration is achieved.

AR stable.Dept rec'ds Opr continue to monitor/maintain well site to ensure final restoration is achieved.At 0915, contacted Chevron rep C. Small via phone to discuss recent earth disturbance activities referenced above and well tag concerns.Mr. Small stated that lines were buried last week in order to vent annuli to prod tanks as annuli previously vented to temporary tanks. Dept rec'd area of earth disturbance is re-rocked to ensure stabilization is maintained.Mr. Small also stated new well tags will be installed immediately.

As per previous conversation with Operator representative Curtis Small, once weather permits, current plan is to re-rock well site to ensure stabilization is maintained in response to recent earth disturbance activities conducted around wellheads/production equipment (record 2227703).
