2017855 | 2011-11-09 | No Violations Noted | Site visit was in response to incident notification by Nick Staffieri (Chevron) to Ed Ritzer (PA DEP Water Qualiy Specialist Supervisor at 07:43 on 11-9-11.Spill occurred do to the following: Haliburton had been cementing the well. Well was being cemented at 8 gpm. This rate caused an overflow of oil based mud at the well head. This overflow was not immediately recognized. Multiple levels of containment were breeched and the mud made it to the surface of the pad. A small area of stone on top of felt lined material was contaminated with this material. Most of the mud had been removed at the time of inspection either by vac'ing or through absorbents. Crews continued to lay absorbent material on any spilled material that had not yet been removed. Absorbent material was then being removed for proper disposal. | |

All areas containing liquids and or hazardous materials secured on containment. Noted that pit levels are approaching 2ft of required lined freeboard. Department spoke to Company man(Brandon) onsite and recommended that pit be pumped in order to prevent a possible pollution event. Also spoke with Chevron Regulatory Compliance personnel via telephone regarding pit freeboard concern. Temporary bmp's installed and functioning properly. Operator shall continue to closely monitor and maintain pit freeboard levels as well as bmp's. No accelerated erosion or sedimentation issue noted. Access road stable. Site gated and protected by security guard. No violations noted during this inspection.

Today, spoke with Company man Brandon whom stated the release (he believes) was actually < 1 barrel occurred while pumping mud from frac tanks into shaker. Due to temperature change mud became viscous and clogged pump which back fed mud and ultimately lead to release onto containment. All released mud is believed to have been contained on secondary containment &immediately removed. No evidence of adverse environmental impact noted at time of this inspection. Based on above statement and visual inspection it appears that all mud was contained and immediately disposed of. Although some areas of the containment are missing berms and/or berms are in need of maintenance the area of release appears structurally sound and appears mud contained on containment. Well site Id sign, ES, ESCGP-1, and PPC plan present. Well tags present. Well(s) drilled not yet in production. One pit located onsite with sufficient freeboard. Spoke with Chevron compliance rep Curtis Small, whom stated that it is believed a check valve on the pump truck failed and while sitting on a grade the diesel line released approximately 3-5 gals of diesel on pad until truck driver realized a line was leaking. Driver immediately addressed the faulty valve/line, and operator began immediate cleanup. A dike was dug to contain release and then a visual cleanup of diesel stained areas began. During today’s inspection, operator had removed contaminated soils and placed in roll off box located onsite, it was also noted that environmental services lab currently onsite collecting soil samples to ensure all contaminated soils have been removed. Chevron rep stated that a containment dike has been built surrounding release. The area has been covered with secondary contain and foam triangles, the contaminated "snow" will be placed within dike and covered. The area is highly marked to prevent third party disturbance and or coverage due to additional snow. On 2/6/13, contractors plan to remove impacted soils, place in sealed drums until final dispostion. Per opr spill quickly contained, minimal environ impact. Written response will be provided on what happend and clean up measures undertaken. As per onsite contracting crew (Penn Mechanical) plans were to removed contaminated snow pile (placed on and covered with secondary containment liner) and place into barrels prior to final disposition. According to reps onsite plans were to remove snow later this afternoon.
